Job Summary

Beacon House is seeking a compassionate and motivated Band 5 Registered Nurse to join our nurse-led clinic supporting people experiencing homelessness and severe social exclusion.

This is a rewarding opportunity to deliver high-quality, person-centred healthcare within a trauma-informed, non-judgemental environment. You will work as part of a multidisciplinary team to support individuals with complex physical health needs, significant health inequalities, and barriers to accessing mainstream healthcare services.

Main Duties of the Job

• Deliver safe, effective, and evidence-based nursing care within a nurse-led clinic setting

• Undertake holistic clinical assessments, observations, wound care, and routine health checks

• Support the management of long-term conditions, including respiratory disease, cardiovascular risk, and infections

• Administer vaccinations and medicines in accordance with PGDs, Patient Specific Directions, and local protocols

• Provide smoking cessation support and health promotion interventions

• Identify and escalate safeguarding concerns in line with local and organisational policies

• Support engagement with guests who may be anxious, disengaged, or distrustful of healthcare services

• Work collaboratively with GPs, nurses, outreach teams, housing providers, and voluntary sector partners

• Maintain accurate, timely, and confidential clinical records using SystmOne

• Contribute to service development, audit activity, and quality improvement initiatives

• Maintain professional standards in line with the NMC Code of Conduct

Person SpecificationEssential

• Registered Nurse (Adult) with valid NMC registration

• Experience working in primary care, community nursing, or acute healthcare settings

• Excellent communication and interpersonal skills

• Ability to work independently and as part of a small multidisciplinary team

• Commitment to equality, dignity, and trauma-informed care

• Confident using electronic patient record systems

Desirable

• Experience working with inclusion health groups or vulnerable adults

• Experience delivering vaccinations and/or smoking cessation services

• Knowledge of adult safeguarding and health inequalities

• Experience working in nurse-led clinics, outreach, or non-traditional healthcare settings
