Job summary
We have an exciting opportunity for a Care Coordinator to support in our clinical service delivery. Many of our clients have complex needs and this role ensures that clients acquire the support they need to receive multi-agency services.
Main duties of the job
The role involves assessment, care planning and provision of stabilisation support to our most complex clients receiving our services. Working together with our therapists, and legal and welfare staff as part of a truly multi-disciplinary team, the ideal candidate will be a Registered Mental Health Nurse, Social Worker or Occupational Therapist with significant experience of working in the mental health sector with complex mental health presentations and will have a passion for working with asylum seekers, refugees and survivors of torture.

- To carry out complex holistic assessments for those individuals who meet the referral criteria of Freedom from Torture.
- To ensure that all care offered is evidence based and all interventions to take into account any specific physical, cultural, psychological, and spiritual and age related needs.
- To report all relevant information to the referring agent and keep them informed of the clients progress.
- To ensure sound risk assessment of all service users to inform levels of care required and which highlight risk to self and others.
- To manage, formulate and implement treatment plans, delivering care under the rehabilitation model, in collaboration with service users, their families, and other significant persons in care planning and management.
- To support clients to understand and access the different pathways available within the service.
- To evaluate effectiveness whilst working within the framework of rehabilitation.
- To work closely and collaboratively with other members of the team to provide optimal care.
- To work collaboratively with other service providers health, social/community and voluntary, including using social care procedures to directly request community services when appropriate.
