Job summary

At Glenridding Health Centre, we are seeking a Practice Delivery Lead who is reliable with an abundance of enthusiasm to join our well-established team.

Main duties of the job

The Practice Delivery Lead is to be responsible for the high quality delivery of the services in the practice, ensuring the delivery of optimum care to the patient population.

We require a self-motivated, disciplined individual who will take ownership of the efficient day-to-day running of the practice, and working as the lead member of the practice team. The post holder will offer strong leadership, acting on practice and business opportunities.

Communicating and Working with Others

The post holder will have responsibility for ensuring that all methods of communication at the practice operate effectively to the benefit of patients and staff. This will include:

  • Working with others including all stakeholders (management team, clinicians, staff team, carers, patients and colleagues both within and outside the practice).
  • Championing interactive communication through all means including written and verbal, meetings, email, patient information leaflets, practice web site etc.
  • Planning, scheduling and convening/chairing meetings, ensuring agendas are prepared and that attendance, minutes noted and actions are taken.
  • Produce patient leaflets as required, ensure that existing patient information is correct and up to date and ensure that reception area and clinical rooms have a supply of leaflets
  • Maintain staff and patient notice boards

People Management

Responsibility for ensuring that teams are led, and work, effectively, that work is appropriately delegated and results delivered, and that HR processes are applied appropriately and in line with current legislation. This will include:

  • Ensuring team members have a clear understanding of work objectives, are committed and motivated to work together as a team.
  • Maintaining team performance even at times of absence or shortage, redeploying staff as necessary.
  • Delegating work activities to team members as appropriate.
  • Recruiting staff in a manner that complies with current legislation, following CHoC recruitment and selection policy.
  • Monitoring and highlighting workforce planning issues.
  • Working with the Primary Care team to ensure good skill mix so that tasks are undertaken by team members with appropriate competence/training and achieves maximum value for money.
  • Providing clear feedback to team members on their performance and allowing the chance for the team member to respond to the feedback through appropriate supervision, appraisal and on an ad hoc basis.
  • Monitoring individuals performance, recording the information and supporting them in improving their performance.
  • Monitoring and manage sickness and annual leave across practice staff
  • Overseeing coordination of the placement of GP Registrars and medical students
  • Providing pastoral support, coaching and/or mentoring as needed.
  • Managing formal HR processes within the practice including disciplinary and grievance, in line with CHoC policy.

Access to Services

Responsibility for ensuring that patients are able to access the services of Glenridding Health Centre in accordance with agreed standards. This will include:

  • Monitoring appointment availability both in advance and on a daily basis and ensuring appropriate action is taken, for example liaising with the rota team to book locum cover.
  • Working with the rota team to ensure adequate staffing levels
  • Planning well ahead for absences such as annual leave.
  • Regularly reviewing access arrangements, for example demand surveys, access audits and recommending changes.
  • Monitoring and developing patient services, alongside the primary care team.
  • Ensuring service development and delivery is in accordance with contractual and local/national guidelines including commissioning
  • Monitoring and maintaining registration policies, monitoring patient turnover and list size.
  • Ensuring that the delivery of practice services is in accordance with equality legislation and protects all patients and carers rights.
  • Promote and support the involvement of the Patient Participation Group

Operations

  • Be responsible for organising and keeping up-to-date a manual of practice protocols
  • Ensure that all administrative systems are running smoothly and recommend any necessary improvements
  • Be responsible for maintaining practice inventory of equipment and organising repair and replacement where necessary
  • Responsible for stock control of stationary and toners and order supplies of stationary and toners as required.
  • Maintain registration policies
  • Organise the annual patient survey, collate results and feedback to the practice team

IT and information

Responsibility for ensuring that the practices IT resources (hardware, software and data) are effectively managed and maintained to protect the integrity of patient records and other records, ensuring that resources are used to enhance patient care. This will include:

  • Ensuring that data and information are received at the correct time, in the correct format, are processed accurately in an agreed format, are appropriately actioned, are stored securely and are transmitted in a way that ensures confidentiality.
  • Liaising with the Digital team to ensure the practices compliance with the General Data Protection Regulation and Freedom of Information Act, other relevant legislation and professional information management standards such as GMC guidance.
  • Implementing and monitoring systems which ensure that patient confidentiality is protected at all times by all staff, who are aware of their responsibilities.
  • Monitoring, evaluating and adjusting the ways in which technology is used in order to benefit patients and the practice.
  • Promoting the use of online services to the benefit of patients and the practice team.
  • Supporting the ongoing maintenance of the practice website and social media.
  • Ensuring that the use of digital tools and systems improves efficiency and that all staff are trained in the use of these tools that will best help them fulfil their role
  • To provide support and training for current and new staff in resolving simple problem with PCs and printers.
  • To liaise with IT support department to resolve other hardware and software issues.

Practice Contracts

  • Project manage the targets associated with APMS contract using the IT system, circulate task lists and follow up on action points.
  • Ensure that all relevant paperwork concerning enhanced services is returned in a timely manner
  • Use Open Exeter/CQRS to monitor performance in Quality and Outcomes Framework (QOF), childhood immunisations, cervical and breast screening to ensure practice meets targets and take remedial action if practice achievement falls
  • Support the Head of Primary Care in liaising with the Eden Primary Care Network and aiding in the achievement of any performance indicators.

Managing Risk

Responsibility for ensuring that potential risks to the practice, including those concerning health and safety of individuals, are identified, addressed and minimised in line with relevant legislation and best practice. Working with the Head of Operations and CHoCS Health & Safety Advisers, this will include:

  • Reviewing policies and procedures, ensuring compliance with professional and legal requirements and guidelines
  • Working with the Estates Lead to Identify risks involved in work activities, management of risks and support to others in management of risks including the delivery of appropriate training
  • Monitoring work areas and practices to ensure they are safe and free from hazard and that they conform to health and safety legislation.
  • Liaising with the Finance team to ensure all insurances are appropriate and up to date.

Primary Care Network

  • With the primary care team, ensuring that the practice team understand the PCN vision aims and objectives
  • Working with practices in the PCN to develop services and deliver agreed objectives
  • Supporting the additional roles reimbursement scheme (ARRS) to ensure additional staff are supported and developed within the PCN and that this resource is equitably shared between participating practices, and optimally utilised by the team at Glenridding Health Centre.
