At Prospect Hospice, we believe that everyone deserves outstanding, compassionate care at the end of life. Our Single Point of Contact (SPoC) Team is the first point of contact for patients, families and healthcare professionals seeking specialist palliative care advice and support.
We are looking for an experienced and motivated SPoC Clinical Practitioner to join this innovative team providing expert clinical triage, holistic assessment and timely guidance to ensure that patients receive the right care, at the right time.
This is a dynamic and patient-facing role ideal for someone who thrives on variety, clinical responsibility, and meaningful impact.
Hours: 15 hours per week, Monday to Friday between 8am and 6pm
Main duties of the job
As a SPoC Clinical Practitioner, you will play a vital role in providing a central, telephone-based access point for hospice care and clinical advice.
You will:
- Receive and assess the urgency of hospice referrals and clinical enquiries.
- Provide clinical advice and emotional support through the advice line by responding to calls and offering high-quality, evidence-based guidance to patients, carers, and healthcare professionals.
- Maintain accurate and confidential patient records.
- Work collaboratively with multidisciplinary teams across primary and secondary care.
- Participate in clinical governance, audits, and service improvement projects.
- Contribute to training and education of internal and external health and social care professionals.
- Support advance and anticipatory care planning, always keeping the patient at the centre of decision-making.
What we are looking for;
You will be a Registered Nurse or Allied Health Professional with the relevant years post-registration experience, ideally with knowledge or experience in palliative care, community nursing, or acute care settings.
You will bring:
- NMC or HCPC registration (e.g., Registered Nurse, Paramedic, or Allied Health Professional).
- Excellent communication and triage skills.
- Confidence in clinical decision-making and risk assessment.
- A compassionate, patient-centred approach.

Prospect Hospice is an independent hospice supporting a community of 300,000 people in Swindon, Marlborough and North Wiltshire, and the villages of Lechlade and Fairford in Gloucestershire.
It is the only dedicated provider of specialist end of life care services within it's locality. The Hospices multi-professionals teams support people in the community, in care homes, at the Great Western Hospital and at the Hospice in Wroughton. Care is also extended to the families and friends of patients.
