Job summary
An exciting opportunity has arisen in our therapy team for a Motor
Neurone Disease (MND) Healthcare Assistant with excellent organisational skills
and clinical experience to provide person centred care to individuals in
various settings throughout the local community.

Hours: 22.5
hours Monday Friday (with some flexibility required to meet patient needs)

Main duties of the job
What is the role?
As a MND Healthcare Assistant
you will be a point of contact for people living with MND and their carers. You
will be providing support, advice and signposting, working alongside the
multidisciplinary team.
In conjunction with the MND
Practitioner you will coordinate the assessment, treatment and support for
individuals living across Swindon. You will be a key player in ensuring the
individuals are able to meet their goals by taking into account their complex
needs and desires and ensuring the appropriate interventions are in place
through your effective communication with other healthcare professionals.
You will have the support of an
amazing and supportive team with opportunities to build and develop your career
including educational courses to develop your skills. An insight into palliative care is ideal and a sensitive communication
style is essential. Relevant training in MND will be provided. If you are
an experienced Healthcare Assistant looking to develop your coordinating skills

Prospect Hospice is an independent hospice supporting a community of
300,000 people in Swindon, Marlborough and North Wiltshire, and the villages of
Lechlade and Fairford in Gloucestershire. It is the only dedicated provider of
specialist end of life care services within its locality. The Hospices
multi-professionals teams support people in the community, in care homes, at
the Great Western Hospital and at the Hospice in Wroughton. Care is also

The prime purpose is to support the MND practitioner in carrying out assigned tasks involving care coordination and ensuring continuity whilst achieving the goals in caring for the complex effects of motor neurone disease. This will involve visiting and providing support and interventions in the patients own home or dwelling.
