Main duties of the job
Locala is seeking an experienced, motivated, and compassionate Registered Nurse to join our Community Nursing Network as a Senior Community Nurse.
This is an excellent opportunity to take on a leadership role, supporting our teams to deliver exceptional care across North Kirklees and Greater Huddersfield.
As a Band 6 Senior Community Nurse, you will combine clinical expertise with leadership skills to ensure safe, effective, and person-centred care. You will act as a role model, mentor, and decision-maker, supporting colleagues and driving improvements in patient outcomes.
Key responsibilities will include:
- Leading and coordinating care for complex patients, ensuring high standards of clinical practice.
- Providing guidance, supervision, and mentorship to Band 5 nurses and wider team members.
- Delegating tasks effectively to ensure efficient use of resources and timely care delivery.
- Supporting service development initiatives and contributing to quality improvement projects.
- Acting as a point of escalation for clinical decision-making and safeguarding concerns.
- Promoting self-care and independence for patients with long-term conditions.
- Collaborating with multidisciplinary teams to prevent unnecessary hospital admissions.
