Main duties of the job
An excellent opportunity has arisen for a Band 6 Senior Nurse post to work within the Community Nursing Night service in Locala, which cover areas across North Kirklees and Greater Huddersfield.
The opportunity will enable the successful applicant to help lead and develop the Night service and team alongside their Band 6 colleagues and the Band 7 team manager ensuring the values of the Organisation is upheld.
An enthusiasm to support our teams to deliver high quality services is a must for this role, and you will also need to demonstrate excellent communication and interpersonal skills with the ability to be adaptable, reliable, and approachable.
The applicant will require a good, positive attitude, the ability to work alongside multi-professionals and their daytime colleagues and the ability to adapt to the needs of the service. The Senior Nurse will also be responsible for providing clinical advice to colleagues at the single point of Contact call Centre and liaising with the on-call manager for Locala as required Out of Hours.
Working within the Community Nursing Night team, you will also undertake duties involving delivery of nursing services and holistic assessments of patients health needs using a person-centred approach.
This includes providing quality and seamless nursing care to housebound and sometimes non housebound patients and being proactive in coordinating their care needs as appropriate, avoiding unnecessary admission to hospital or long-term care institutions.
