Main duties of the job
Are you a confident nurse with community experience who wants to work with patients in an acute setting to maximise their independence? An excellent opportunity has arisen for an enthusiastic and motivated nurse to join Locala's new Self-Management Service.
The self-management service works towards the NHS' Long-Term Plan for personalised care, patients will be reviewed both at home and virtually whilst working towards becoming experts in their own health care interventions.
Your main role will be to support learning and competence for patients ensuring that they feel confident and able to optimise their skills, maximising the benefits of self-management,
The successful candidate will:
- Be a nurse with community experience who is passionate about patient's choices, their abilities to self-manage, and personalised care.
- Build new and effective working relationships with colleagues who work within the acute setting.
- Be willing to undertake any further clinical skills and health coaching training that will be required for the role.
- Act as a role model, using their skills to support patients in an acute setting, building confidence patients they feel able to continue with these skills at home.
- Demonstrate flexibility in their approach, allowing for regular evaluation and adaptation of the service during its infancy.
