Sexual Health Nurse with a specialist interest in outreach delivery and marginalised groups
Main duties of the job
The post holder will provide a high standard of care to patients accessing the clinical hub but will focus on the outreach/community delivery ensuring that those populations most at risk of sexual ill health are offered sexual health care.
The post holder will work collaboratively with team leaders, ANPs and service leads to develop and implement community outreach clinics in a variety of venues promoting access for vulnerable adults and young people.
This will include looked after children, those effected by substance misuse, sexual exploitation, those involved in the sex industry, men who have sex with men and those with complex health and social needs. The post holder will work in collaboration with key partners and stakeholders across the district to identify target areas for sexual health interventions and they will support the development, implementation and evaluation of these interventions with the support of the integrated sexual health team and the outreach service.
The successful candidate will be innovative and ambitious, and they will be able to demonstrate an ability to work efficiently, autonomously and productively to make a positive impact on patient outcomes. We require candidates who will contribute to the overall improvement of the health and wellbeing of our patients by delivering care through a patient-centered approach and who will join a friendly and progressive team making a difference to local communities and reducing health inequalities.
