Main duties of the job
Assistant Practitioner Integrated Transfer of Care (ITOC)
Band 4: £26,530 - £29,114
21 hours per week

Locala ITOC provides comprehensive care for adults in their own homes, community rehabilitation facilities, and care homes.We have an excellent opportunity for an experienced Assistant Practitioner to join our team.The role is based across North and South Kirklees team, we require you to be flexible in terms of service needs, and there is an expectation that you can travel Kirklees wide.
The Assistant Practitioner will provide rehabilitation and support within the ITOC team, in a variety of settings.This role requires a motivated and dynamic person to work within our established service, and it is essential that you have experience in working as part of a multidisciplinary team.
You will have experience of treating patients with complex disability/orthopaedic conditions/delirium dementia and falls. Experience of being flexible and working in a variety of unplanned care settings i.e. A&E, with Frail patients in the community and Discharge support in the Acute Hospital and in our Recovery hub.
A formal Assistant Practitioner qualification is desirable, but at least willingness to work towards this is essential.
