We are recruiting people with a passion for individualised holistic care, the ability to work both autonomously and together within our developing multidisciplinary team. As part of the team you will deliver individualised care for patients families and carers supporting their wishes and preferences at home.

Experience in Palliative Care is desirable; however, we welcome applications from health professionals with post registration experience who are committed to providing excellent care and are keen to learn and develop new skills in Palliative Care.

The post holder will be required to work across shifts covering the period overnight. Although based in the community, post holders may occasionally be required to work in other Rennie Grove Peace Hospice Care settings in order to ensure adequate staffing levels and to gain experience of the wider hospice delivery of care (e.g. Inpatient Unit and outpatients teams). As this role delivers services in the community it's essential that you are a car driver and have access to vehicle for use.

  • Salary: £34,685 - £41,798 per annum (pro rata)

Main duties of the job

The main duties of the role will include:

  • Proactively contributing to the delivery of high quality safe evidence based care.
  • Delivering high quality Specialist Palliative and End of Life nursing care as part of the In- Patient Unit multidisciplinary team.
  • Leading on and coordinating assessment, planning and evaluation of individualised patient centred care. Ensuring a holistic approach that is sensitive and responsive to the patients ever changing needs.
  • Being responsible for the coordination and smooth running of a shift and visits ensuring it is well organised.
  • As a member of the wider Rennie Grove and Peace Hospice Care staff, sign up and demonstrate delivery of the organisational agreed mission, vision and values.

Job description

Job responsibilities

The main duties will include:

  • To undertake a holistic needs assessment that explores individual patients and their carers/families physical, emotional, spiritual, practical, welfare right and if appropriate the families pre and post bereavement needs, using a variety of tools to support the process.
  • To be able to develop, update and monitor the individualised care plans ensuring they are patient centred and support the identified patient needs, including ongoing referrals to other services as required.
  • To be able to deliver both generalist basic and complex specialist care interventions that is responsive to the individual patients requirements.
  • To be able to communicate highly sensitive information, ensuring patients and their families/carers are adequately supported.
  • To be able to demonstrate through electronic and paper documentation, and verbal communication the care undertaken and patient outcomes.
  • To be responsible in leading on initial Holistic assessment and reviews of care when required and discharge planning from service.
  • To be an active member of the MDT acting as the patient advocate contributing to the presentation, decision making of the patients care planning.
  • To be able to communicate effectively to own team extended team and outside agencies, ensuring continuity of care is maintained across the different care settings/ home.
  • To follow all agreed clinical procedures and statutory regulations related to Medicine management.
  • As a hospice paramedic of identified shifts be responsible for dealing with any service enquiries, calls and referrals as required.
  • To signpost and inform patients and their families/carers of the range of services available to them, external resources and day to day information relating to the service.
  • To have the highest regard for the principles of confidentiality and the importance of patient consent, ensuring adherence to the hospice data collection and patient consent policy.
  • In accordance to all hospice policies, best practice guidelines and clinical protocols ensure own and others practice is safe and effective.
  • To participate in keeping accurate and legible records in accordance with existing practice and procedure at the Rennie Grove Peace Hospice Care. To be aware of the legal requirements associated with keeping such records.
  • To deal with any initial complaints and conflict as required, in a polite, calm manner providing the information and support required and report to line manager in line with the Rennie Grove Peace Hospice Care processes.
  • To be accountable for reporting any risk, hazards or accidents that may occur in the service area, and ensure compliance with the terms of the Health and Safety at Work Act.
  • To follow all agreed policies including manual handling, infection control, health and safety, safeguarding, ensuring a safe environment for patients, visitors and staff visiting and within the service.

  • Non-medical prescribing Qualification or willingness to complete
