Hours: 37.5 per week (part time considered)

Location: Watford Peace Hospice (Travel between all Rennie Grove Peace offices will be required occasionally as part of this role.)

Salary: Rennie Grove Peace Band 4: £26,530 to £29,114 per annum (pro rata)

The role of Nurse Associate is to Deliver high quality Specialist Palliative and End of Life nursing care as part of the In-Patient unit multidisciplinary team.

Main duties of the job

The role of Nurse Associate is to Deliver high quality Specialist Palliative and End of Life nursing care as part of the In-Patient unit multidisciplinary team.

You will therefore need to demonstrate your abilities in the following areas:

  • To support the Registered Nurse in ensuring the delivery of high-quality patient centred care, ensuring a holistic approach that is sensitive and responsive to the patients ever changing needs.
  • To ensure Rennie Grove Peace organisational vision and values are integral to the care delivered and role undertaken.
  • As the named nurse provide and monitor the holistic nursing care for an allocated number of patients each shift.

At Rennie Grove Peace Hospice Care, our unique care provides a lifeline of compassion and support to patients, carers, and families facing progressive life-limiting illness. We provide palliative and end of life care support, including bereavement services, within Buckinghamshire and Hertfordshire and our patient services are available to adults, as well as patients transitioning into adult services (16 to 25 years.) Bereavement support is offered for all ages to families of patients and those around them, as well as throughout the community, in schools and workplaces.

We are proud to offer care in the place that best suits the individuals needs, whether thats at home, in a care home, in a short stay in our inpatient unit, out in the community, or in our purpose-built Living Well Centre facilities. As a hospice care charity, we have offered this support, free of charge, for over 40 years.

Our staff team of 340+ includes a wide range of patient-facing disciplines plus operational support based in Watford, Tring, St Albans, Chalfont St Giles and Berkhamsted. The Charity is now one of the larger UK hospice charities, reaching a GP population of 797,037 of whom 75% live in Hertfordshire and 25% in Buckinghamshire.

Job responsibilities

To undertake a holistic needs assessment that explores individual patients, and their carers/familys physical, emotional, spiritual, practical, welfare rights and, if appropriate, the familys pre and post bereavement needs, using a variety of tools to support the process.

To be responsible for patient assessment, planning of care, giving and supervising safe and compassionate care, and evaluating the effectiveness of care under the supervision of the registered nurse you are working with.

To work with the registered nurse to develop, update and monitor the individualised care plans ensuring they are patient centred and support the individual identified patient needs.To be able to deliver basic and complex specialist palliative and nursing care interventions that are responsive to the individual patients.

Have knowledge of common palliative and EOLC symptoms and management strategies with experience to anticipate changes and knowledge of when to seek advice.

To be able to communicate highly sensitive information, ensuring patients and their families/carers are adequately supported.

To act as a role model for students and junior staff, where applicable act as their mentor.

To have the highest regard for the principles of confidentiality and the importance of patient consent, ensuring adherence to the hospice privacy statement and patient consent policy.

In accordance to all hospice policies, best practice guidelines and clinical protocols ensure own and others practice is safe and effective.

To regularly attend service meetings, team meetings and other intra hospice meetings, ensuring information is cascaded to colleagues accordingly.

To represent team as required at professional meetings as requested and feedback information appropriately.

To participate in keeping accurate and legible records in accordance with existing practice and procedure at the Rennie Grove Peace.

To be aware of the legal requirements associated with keeping such records.

  • Educated to GCSE, including Maths and English
  • IT experience and ability to input into the electronics record system following induction training
  • Knowledge of when to seek advice and refer to a registered health or care professional
  • A good understanding of technology with a working knowledge of Microsoft office packages i.e. Word, Excel and Outlook.
  • Able to build good working relationships.
  • Able to communicate effectively both verbally and in writing.
  • Good administrative and record keeping skills and able to work with good attention to detail.
  • Strong organisational skills.
  • Sensitivity and understanding to the needs of the volunteers and customers, many of whom may have experienced Rennie Grove Peace services or be bereaved themselves.
