- Role: Health Care Assistant in Rapid Personalised Care Service (RPCS) Fixed Term Contract
- Location: Rennie Grove Peace - Tring
- Salary: £22,816 - £24,336 per annum (pro rata). Both full and part time hours are available.

We have opportunities for an enthusiastic and compassionate Healthcare Assistant to join our friendly Rapid Personalised Care Service team. This is a community based role working with our patients, in their own homes.
The successful candidate will work as part of our dedicated team providing high standards of care for our patients and those people who are important to them.
Although based in the RPCS community team at Rennie Grove Peace - Tring, post holders may occasionally be required to work in other Rennie Grove Peace Hospice Care settings in order to ensure adequate staffing levels and gain experience of the wider hospice delivery of care.
Main duties of the job
Key objectives:
- Proactively contribute to the delivery of high quality safe evidence based care.
- Deliver high quality Specialist Palliative and End of Life care as part of the Rapid Personalised Care Service team.
- Competently working alone, as required, with patients and their families in the home.
- Working collaboratively with hospice and community partners.
- Acting as a Rennie Grove Peace Hospice Care ambassador, actively demonstrating our values and positively promoting the specialist care we provide.

Rennie Grove Peace Hospice Care services are provided 24 hours a day, 7 days a week.
The post holder will work a varied shift pattern (between 7am-11pm) on a rota basis from Sunday to Saturday. Post holders may occasionally
be required to work in other Peace Hospice Care settings.
Key objectives:
- Demonstrate and maintain consistently high standards of individualised personal care and support under guidance of the RPCS Management.
- Work alongside the patients families and/or health care professionals, maintaining an approach that incorporates physical, psychological, social and spiritual aspects of care in accordance with the Rennie Grove Peace philosophy.
- Observe and report changes in the patients symptoms, conditions and circumstances to the RPCS Management and notify other healthcare professionals as necessary.
- Work in accordance with the patients agreed care plan.
- Responsible for identifying and acting on any risk of safeguarding to patients and their families in accordance with the Rennie Grove Peace policies.
- Keep accurate and legible records in accordance with legislative requirements and Rennie Grove Peace practice and procedure.
