Coordination Centre Clinical Nurse Specialist

- Salary: Rennie Grove Peace Band 7 - £46,148 - £52,809 per annum (pro rata)

Main duties of the job
The purpose of this role is to be part of a small team of senior nurses who are responsible for the clinical coordination of care for patients and families referred to Rennie Grove Peace.

At Rennie Grove Peace Hospice Care, our unique care provides a lifeline of compassion and support to patients, carers, and families facing progressive life-limiting illness.
We provide palliative and end of life care support, including bereavement services, within Buckinghamshire and Hertfordshire and our patient services are available to adults, as well as patients transitioning into adult services (16 to 25 years.) Bereavement support is offered for all ages to families of patients and those around them, as well as throughout the community, in schools and workplaces. We are proud to offer care in the place that best suits the individuals needs, whether that is at home, in a care home, in a short stay in our inpatient unit, out in the community, or in our purpose-built Living Well Centre facilities.
As a hospice care charity, we have offered this support, free of charge, for over 40 years. Our staff team of 340+ includes a wide range of patient-facing disciplines plus operational support based in Watford, Tring, St Albans, Chalfont St Giles and Berkhamsted. The Charity is now one of the larger UK hospice charities, reaching a GP population of 797,037 of whom 75% live in Hertfordshire and 25% in Buckinghamshire.

You will therefore need to demonstrate your abilities in the following areas:
- Being the point of contact for patients, families, clients and professionals who contact the hospice, dealing with all clinical enquiries.
- Providing clinical oversight and triage for referrals to ensure that the patients and family members get the right Rennie Grove Peace service in a timely manner.
- Developing the Coordination Centres nursing role to ensure it provides a responsive, evidence based and high-quality service to beneficiaries and professionals.
- Working as a Specialist Palliative Care CNS in a different Rennie Grove Peace clinical setting on a monthly basis to maintain and develop skills.
- The post holder will undertake activities such as audits and case review in order to improve the Coordination Centre Service.
- Contributing to the wider strategic development of the hospice and help to translate local and national strategy relating to hospice care and Rennie Grove Peace to implement and deliver an effective service.
- Act as a Rennie Grove Peace ambassador within the community.

- Evidence of continuous personal and academic development.
- Proven palliative care experience.
- Proven experience of managing people/strong clinical leadership.
- Proven experience of effective team working.
- IT literate including a good knowledge of Excel Word, a clinical IT system.
- Excellent written and verbal communication skills.
- Good organisational skills.
- Teaching and presentation skills.
- Advanced assessment skills and specialist symptom control knowledge to plan, implement and
- evaluate nursing care.
- Drive and enthusiasm with a positive approach.
- Caring and empathetic approach
- Flexible approach and attitude to working.
- Commitment to Rennie Grove Peace hospice car emission and aims.
- Sensitivity and understanding to the needs of your team and customers, many of whom may have
