Job responsibilities

• Demonstrate clinical excellence and maintain consistently high standards of clinical practice in palliative care.
• Act as a role model providing clinical leadership for colleagues.
• Advise and support on patients with complex needs and their families at home, maintaining an approach that incorporates physical, psychological, social and spiritual aspects of care, in accordance with the organisation mission and values.
• Manage a caseload of complex patients promoting the highest possible standards of individualised care, symptom management and bereavement support through a process of assessment, planning, coordination and evaluation.
• Effectively allocate and support team members to implement the care plan, monitoring and evaluating the care provided, delivering practical nursing care when appropriate.
• Monitor and prioritise team referrals.
• Ensure all written and verbal communication is accurate and legible in accordance with NMC and the hospice policies and guidelines.
• As a Non-Medical Prescriber, adhere to prescribing guidance for palliative care patients adhering to Rennie Grove Peace Hospice Care policies and guidelines.
• To demonstrate leadership by means of effecting change within own sphere of practice when and where indicated.
• Making recommendations to improve services and act as a specialist resource for other professionals - Influencing, motivating, supporting and enabling others to contribute towards the effectiveness and success of the organisation.
• Participate in service reviews and the development, implementation and evaluation of Rennie Grove Peace Hospice policies, as required to drive service improvement.
• Maintain record of equipment borrowed and returned Effectively allocate, support and monitor the work of the team, including Bank Nurses, in collaboration with other team Clinical Nurse Specialists/ Clinical Paramedic Specialists.
• In collaboration with the Head of Community Services for the locality, manage performance and professional development of the Hospice at Home team to support planned and unplanned care.
• Act as a co-mentor for new staff and oversee successful completion and timely sign off of their baseline competencies.
• Carry out individual 1 to 1 and performance review process (annual and mid year reviews) as instructed and complete personal development plans.
• Within the context of the role of a Clinical Nurse Specialist/Clinical Paramedic Specialist, act a skilled educator who is able to advise patients, families and multi-professional colleagues appropriately
• Participate fully in the orientation/mentorship of new staff, working in collaboration with the Head of Community Services to support staff through the probationary period
• Supervise and teach Rennie Grove Peace Hospice trained staff Provide specialist palliative care training to multi-disciplinary team colleagues within the community

Person Specification

Qualifications

Essential

• - First level Registered Nurse or Registered Paramedic
• - Degree Level or modules in Palliative Care or another relevant subject
• - Evidence of continuous personal and academic development

• - Proven palliative care experience
• - Non-medical Prescriber or a commitment to successfully achieve this qualification as agreed with line manager
• - Advanced assessment skills training/commitment to attend this qualification
• - Advanced communication skills training
• - Proven experience of managing people / strong clinical leadership.
• - Proven experience of effective team working
• - IT literate including a good knowledge of Excel, Word, a clinical IT system.
• - Excellent written and verbal communication skills
• - Good organisational skills
• - Teaching and presentation skills
• - Advanced assessment skills and specialist symptom control knowledge to plan, implement and evaluate nursing care
• - Drive and enthusiasm with a positive approach
• - Ability to work autonomously and as an effective member of the team
