Job summary
A qualified nurse who will support the team lead in ensuring that palliative patients receive the highest standard of care. To act as a role model and support the team manager to operationally manage and lead a team of carers who provide care to patients in their own homes. To be proficient in IT skills and to ensure the management of rotas and support the RPCS team manager with monthly reporting requirements with accurate recording of data.
Main duties of the job
To support the team lead in clinical assessment of patients and the management of the caseload.
To support and act as a mentor for carers and liaise with members of the health and social care professional, patients, families and carers.
To ensure accurate recording of data, management of daily rotas and to be part of the out of hrs call rota.
To work flexibly across the organisation.
Acting as a Rennie Grove Peace hospice ambassador within the community.
