Job summary
We are looking for a social worker assistant who has experience of working with adults and their families. You will be providing psychosocial and practical support as necessary, to patients and family members.
Main duties of the job
- To monitor referrals and complete day-to-day tasks as allocated by the Senior Social Worker.
- To manage own caseload, with the support of the line manager.
- Help patients, their carers, and families to establish their needs, and support them to understand how they may access services that can meet their need.
- Advocate for patients and carers as needed with other statutory and voluntary agencies.
- One-to-one interviews and direct work either assessing needs and risk and / or providing emotional support for patients and family carers.
- This work can take place on site or potentially in patients' homes.
- To give basic benefits advice and make referrals to specialist benefits advisors as required.
- To offer social support and pre bereavement support to patients and their carers.
- When in direct contact with patients and carers, offer support, guidance, signposting and referral to services if required including referral to bereavement services after the death of a patient.
- To promote an understanding within the Multi-Disciplinary Team of the social difficulties encountered by patients and their carers to influence the development of their care plans.
- Maintain confidentiality and good professional practice, to ensure that there is the fullest co-operation with external agencies in the patients' best interests.
- To ensure carers of adult palliative care patients are provided with the opportunity to have their needs assessed using the Zarit tool.
