Tennant
Street Medical Practice is situated in Central Stockton with easy access to
the A66 and A19.The
practice is a semi new build with excellent facilities and a friendly
atmosphere.
We
are a large, busy practice with over 14,530 patients and 9 GPs. There is a full nursing team comprising Urgent
Care Practitioners, Long Term Conditions Nurses, Practice Nurses and HCAs. We open 5 days per week with one late
evening.
Due
to the retirement of one of our current nursing staff we are looking to
recruit a Practice Nurse to work between 25 37.5 hours per week. The days will be fixed following agreement.
Main duties of the job
Ideally,
we are looking for a qualified nurse who is experienced in dressings,
immunisations, cervical screening, contraception and long-term condition
monitoring. However, training can be
offered if you are the right person for the job but don't have all the
skills, so sell yourself to us in your application!
The
role requires diplomacy, patience, tenacity and a good sense of humour. This
is a challenging but very rewarding post.
We
are looking for someone who is enjoys working in a dynamic environment, has
excellent customer service skills and who ideally has GP practice experience.
You
need to be a team worker and be able to deal with any situation that arises
with confidence and professionalism.
Excellent
knowledge of SystmOne would be a distinct advantage.

- Registered (first level) Nurse
- Current Registration on NMC Register
- Desirable - post registration experience working in primary care
- Evidence of Ongoing Professional Development.
Key Features of the Role Delivery of nursing care to patients in the practice population
- Direct Clinical Involvement in Long Term Condition and Chronic Disease Management
- Providing advice, support and liaison with other practice staff
Clinical Nursing Practice
- Complex Wound Management and provide support to junior staff members to create wound care plans.
- General support to junior staff members
- Vaccinations and Immunisations
- Infection Prevention & Control and assist with audits
- Emergency Management i.e. anaphylaxis and resuscitation
- Assess, plan, provide and evaluation nursing care to meet the care needs of individuals and groups in the practice population.
- Undertake diagnostic health screening, health surveillance and therapeutic interventions within a broader health promotion/public health strategy
Contraception
- Women's Health including cervical screening, swabs and HRT
Management of Long Term conditions
- Diabetes
- Respiratory Disease Management-COPD & Asthma
- Cardiovascular Disease Management-CHD, Stroke/TIA, PVD & Heart Failure
- Must have business insurance to attend home visits to conduct LTC management and immunisations
- Enable individuals with long term conditions and their carers to make informed choices concerning their health and well-being and to organise their own support, assistance and action.

Travel Vaccinations
- Support individuals with long term conditions and their carers to change theirbehaviour and to reduce the risk of complications
- Manage programmes of care for patients with acute and chronic diseases by planning and evaluating care
- Work across boundaries and in partnership with primary / secondary care clinicians and social services, coordinating care and promoting a multi-disciplinary approach
- Review prescribed medication and provide advice on all aspects of medicines management. Refer to relevant clinicians where appropriate
- Contribute to the development of practice policies and system of practices
