Job summary

We are seeking a Clinical Nurse Specialist to join a new Advance Care Planning Service for care homes in Barnsley, based at Silkstone Health Centre and working with patients across local care homes.

Main duties of the job

This role offers the opportunity to join Marie Curies Advanced Care Planning Service in Barnsley, working within primary care alongside Silkstone Health Centre. The postholder will use palliative care expertise to support patients in care homes with advance care planning and RESPECT forms, contribute to education and training, and work closely with GPs, community services, and the wider palliative and end of life team. The role is autonomous, involves managing a caseload, supporting colleagues, and aims to reduce unnecessary hospital admissions in the last year of life. Regular travel to care homes is required.

Salary:Marie Curie Pay Scale, aligned with AFC Band 6 - £38,638 to £46,581 per annum (increasing to £39,959 to £48,117 per annum from 1st April 2026) + enhancement (Pro Rata)

Hours/Contract: Full Time/Part Time core hours 8.30 4.30 pm

This is an excellent opportunity for someone looking to join the Marie Curie team in our Advanced Care Planning Service in Barnsley. By sharing your nursing and palliative care expertise, you will have the chance to significantly influence and shape patient care in Barnsley.

This service will be based within Primary Care partnered with Silkstone Health Centre, Barnsley, where you will work with GP's in primary care supporting patients in their allocated care homes to complete advanced care planning and RESPECT forms. You will be integrated into the Barnsley Palliative and End of Life Team and will provide education to care homes in collaboration with the care home Clinical Nurse Specialist. The aim of the role is to prevent unnecessary hospital admission in the last year of life.

In this key role, you will work autonomously and manage your own caseload. Your responsibilities will also include training and development, working closely with primary care and community services to offer onwards referrals and support for patients and their families and supporting the Healthcare Assistant for this service.

As a confident communicator and calm decision-maker, you will need experience and knowledge in palliative care and community services, along with well-developed advanced communication, assessment, and planning skills. The ability to prioritise your work while managing a variety of changing demands is essential. This role is based in primary care with regular travel to visit patients in care homes. Therefore, the ability to drive and access a vehicle insured for business use is essential.

  • Registered Nurse on NMC Register
  • Experience in Mentorship/Teaching qualification
  • Advanced communication skills and ability to have sensitive and difficult conversations with patients
  • Strong time management skills and the ability to proactively plan and schedule your own workload, to meet patient care requirements.
  • Core hours will be 8.30 am - 4.30pm however there will also be a need for Flexible working around the needs of the patients and their families (Out of hours appointments).
  • Integration with Yorkshire place based teams.
  • An understanding of specific needs of patients approaching end of life and in the terminal phase.
  • Driving license and access to your own vehicle and a willingness to travel within the region
  • This position is funded by the Oak Foundation Service for an initial period of two years, with the potential for funding to be extended.
