We are recruiting a Healthcare Assistant to join our new Care Homes Advance Care Planning Service in Barnsley.

Main duties of the job

An opportunity to join the Care Homes Advance Care Planning Service in Barnsley, working closely with GP's and care homes to support patients and families with meaningful advance care planning. Based in primary care and line managed by Clinical Nurse Specialist, the role involves providing compassionate palliative care support and building strong relationships across community services. This is a community based, post requiring travel to care homes, so a driving license and access to a vehicle are essential.

Job responsibilities

Salary: Marie Curie Clinical Pay Scale Band 2 (aligned with AFC) £24,465 per annum (increasing to £25,272 from 1st April 2026) + enhancements (pro rata)

Contract/Hours: Part Time 22.5 hours per week

Contract Type: Permanent

Based: Silkstone Health Centre, Barnsley

This is an excellent opportunity to join our Care Homes Advance Care Planning Service in Barnsley and make a real difference to people at an important time in their lives.

Based in primary care and working in partnership with Silkstone Health Centre, you'll collaborate closely with GP's to support patients living in local care homes. Using your experience in palliative care, you'll help patients and their families engage in meaningful advance care planning.

Working under the line management of our Clinical Nurse Specialists, you'll provide direct, compassionate support to patients and their families, building trusted relationships across community and primary care services. This role is ideal for a confident and empathetic communicator with a strong understanding of palliative and community care.

As this is a community based role, you'll be required to visit patients in their care homes, therefore driving and access to your own vehicle for business use is essential.

What were looking for:

  • S/NVQ Level 2/3 qualification or an equivalent certificate/diploma is required
  • Relevant community experience, providing care and support for people with life limiting illness or palliative care needs
  • Advanced communication skills with the ability to have difficult and sensitive conversations.
  • Emotional resilience and the ability to work on your own are crucial, though we will of course provide excellent on-going training and support
  • Be a UK licensed driver with access to a car and business insurance is essential
  • This position is funded by the Oak Foundation Service for an initial period of two years, with the potential for funding to be extended
