Job summary
This service operates within Primary Care, working in partnership with GP surgeries across Bradford. Using the NHS early identification tool, you will proactively identify patients who may be in their final year of life but have not yet been recognised as needing palliative care. As the first point of contact, you will provide support, facilitate advance care planning, and guide patients to additional services. Your role will also include visiting patients in their homes and making referrals to hospice, voluntary, or community services as appropriate
Main duties of the job
Working closely with and reporting to the senior nurses within the service, you will provide direct support to patients and their families who have recently been identified as having a palliative diagnosis.
