Job summary
Marie Curie is seeking experienced Senior Nurses to develop a new and exciting palliative integrated discharge service from acute sites across Cwm Taf Morgannwg University Health Board. Apply today for a rewarding career where you can help shape the future of palliative care within the region!
The new service will involve Senior Nurses proactively identifying patients that are approaching the end of their lives, ensuring supportive conversations are had to enable them and their loved ones to discuss their preferences and wishes.
Working with the Health Boardss existing teams and the patients loved ones, the Senior Nurses will ensure holistic assessments are undertaken to support fast track assessments and ongoing care and treatment plans. This will include advance care planning and the completion of clear and comprehensive treatment escalation plans.
Main duties of the job
The Senior Nurses will work as part of the Health Boards discharge team and collaborate with the existing multi-disciplinary and specialist palliative care teams to ensure patients are discharged safely and timely to an alternative care setting according to preference and need.
The Senior Nurses will also work closely with Marie Curies existing Hospice Care at Home Service to utilise our care and support capacity to facilitate discharges into the community.

- Registered Nurse on NMC Register with a relevant Nursing Degree.
- Proven experience in palliative care (hospice/hospital/community).
- Strong communication and team collaboration skills.
- Ability to manage workload and prioritize effectively.
- Commitment to continual professional development and reflective practice.

Based:The Senior Nurses will be based in Ysbyty Cwm Rhondda (Discharge Hub) but will need to be flexible to work across the Health Board area/Hospital sites as required.
