Job summary
We are seeking a compassionate and dedicated Healthcare Assistant to join our community team in Hertfordshire and work towards providing holistic, person centred, end of life care and support needs of people in their own homes.
This is aCommunity-based role in Hertfordshire (Herts Valley), covering Watford, St Albans, Hemel Hempstead,
Tring, Potters Bar, Berkhamsted, Borehamwood, Radlett, Abbots Langley, and
Bushey.
Main duties of the job
Your responsibilities will include monitoring and assisting with the maintenance of patients' skin integrity and personal hygiene, observing and supporting nutritional needs, attending to comfort requirements to facilitate rest and sleep, providing mobility support, and maintaining a clean and orderly care environment.

We want a different society than the one we live in now. Our mission for the next five years is to close the gap in the number of people missing out on what they need at the end-of-life.

As a

Contract: Permanent, Reduced hours
Work Schedule: Night Shifts: Monday to
Sunday, including bank holidays
Shift hours: 10:00pm 7:00am
Salary:£24,465 per Annum, Plus
enhancements
Based: Community-based in Hertfordshire (Herts

What
were looking for:

At least six months of recent experience providing
care in a Health or Social Care Environment, including caring for patients at
the end of life.

NVQ Level 2 in Health and Social Care or equivalent
(preferred, not required).

Be person-centred and compassionate
when caring for the person and those important to them.

Ability to develop and evaluate care
management plans that meet the complex needs of the person, and those important
to them.

Emotional
resilience and the capacity to work independently.

A
valid driving license and access to a car for travel to patient homes are
required.
