Marie Curie is the UKs leading end-of-life
charity. We are the largest non-NHS provider of end-of-life care in the UK, the
only provider across all 4 nations, delivering community nursing and hospice
care across the country, while providing information and support on all aspects
of dying, death, and bereavement. Our leading research pushes the boundaries of
what we know about good end-of-life, and our campaigns fight for a world where
everyone gets to have the best possible quality of life while living with an illness,
they are likely to die from.
The care and support we provide is highly
valued by the people we care for and their loved ones, but at present we are
only reaching around 10% of dying people at the end of life. Right now, one in four
people in the UK with a terminal illness, do not get the care or support they
deserve at the end of their lives.
We are seeking a Nurse Practitioner to join our
community based rapid response team in Northampton to work towards providing
holistic, person centred, end of life care and support needs of patients and
those important to them.
The nurse practitioner will provide support and
advice to patients in their own homes, including care homes, and collaborate
with the CCC, Rapid Response Service, and Out of Hours services. This role
serves as a resource for other professionals on palliative care issues,
including physical, psychological, social, spiritual support, and bereavement
care, ensuring the most appropriate service for patients and their carers.
Within their professional boundaries, the post
holder will deliver specialist palliative care from initial contact, including
history taking, clinical assessment, diagnosis, treatment, and evaluation. They
must ensure interventions effectively support patients, their relatives, and
carers, respecting patients' choices, dignity, and end-of-life wishes.
The nurse practitioner will demonstrate safe,
clinical decision-making and expert care, including assessment and diagnostic
skills for end-of-life care patients. They will work collaboratively within the
team to meet patient needs, lead and support policy and procedure delivery, and
provide nurse leadership as required.
Job Structure:Full
Time; 37.5 Hours per week; The role covers a 24-hour period. This means that
the position will involve working in shifts to ensure continuous coverage
Salary:Marie
Curie Pay Scale Band 6 (aligned with AFC) £37,338 - £44,962 Pro-rated per annum

What were looking
for:
Essential:

Minimum
of 5 years post-registration experience, including at least one year of recent
palliative care/end-of-life care experience.

Experience
in carrying out complex health need assessments.

Evidence
of ongoing professional development in Management of patients
with palliative care and end-of-life needs.
Desirable:

Experience
working in a hospice or in the community at a senior level with palliative care
patients.

Clinical
supervision training and experience.

Community
nursing specialist degree.
