Job summary

An exciting opportunity has arisen for an Admissions and Discharge Co-ordinator to join the team at Marie Curie. You will also get a very rewarding salary package, with a one off signing-on payment equal to 10% of annual base salary recruitment premia plus a retention bonus of 10% of your basic annual salary until 31st July 2024 paid on a monthly basis after completing 8 weeks in post and induction training

Main duties of the job

Anopportunity has arisen to work as part of our multidisciplinary team in the role of an admissions and discharge coordinator. This role will be contributing to the provision of specialist palliative care for patients facing life limiting illness within a hospice setting.

Based Marie Curie Hospice, Hampstead (North London)

As a registered Nurse, you will actas the key co-coordinator for all inpatient admissions and discharges ensuring that these are processed in line with the hospice and national admission / discharge policies and processes. You will be the first point of contact for all hospice referrals and must be able to carry out initial clinical triage to establish if referral is appropriate.

You will work closely with the Inpatient Unit, Support and Well-Being Centre and London Co-Ordination Centre teams to ensure that referrals are processed in a timely and effective manner.

You will be required to maintain accurate data regarding referrals and bed status in order to provide up to date information to referrers. You will participate in daily referrals meetings and our weekly MDT which requires effective communication skills with a variety of healthcare professionals.

You will establish collaborative working relationships with current and potential referrers.

You will be involved in supporting the clinical team with complex discharge planning, being proactive in liaising with social services, CHC teams and nursing/care homes to ensure that patients are discharged safely and without delay

This role will provide you with an opportunity to support a cohort of patients with complex palliative care needs requiring admission to the hospice for a variety of reasons and to play a key role in providing a safe discharge to an appropriate setting. It will also allow you to demonstrate and develop your leadership skills while working with key external partners.
