Are you passionate about palliative and end of life care and looking for
a new challenge? We are looking for someone who holds a high amount of empathy
and compassion, and who can make a real difference not only to our patients but
to their family and support network.
You will seek to provide a high standard of holistic, skilled,
patient-centred, palliative care to all community patients, as part of Mountbatten Hampshire
multidisciplinary community services team, providing nursing care under the direct
clinical leadership of the Locality Lead/ Senior Nurse on Duty. You will need
to be an enthusiastic and motivated nurse, keen to develop both your practice
and education, in line with the role requirements, and have been qualified for
at least 2 years, having gained relevant experience.
You will need to hold a clean
driving licence and have the use of a car.
Part-time: 30 hours a week

Main duties of the job
The Associate Nurse Practitioner role will contribute to the provision of specialist palliative and end of life care in the Community, being based at Mountbatten Hampshire.
Under the supervision of the Locality Leads, they will work autonomously and as a member of the Community Palliative Care Team influencing the delivery of ongoing patient care.

Job responsibilities
To work across care settings providing advice, education and support to patients, their carers and other healthcare professionals, acting as an ambassador with external partners and other key stakeholders.
The role will promote preferred place of care, management of complex end of life care symptoms and managing staff, patient and carer expectations whilst meeting the integrated care needs of the patient.

Develop
and maintain communications with patients, carers, other professionals and
volunteers, developing specialist knowledge in palliative and end of life care.
Communicate information to distressed patients and/or bereaved family members/
people in a sensitive manner.
Communicate
with patients in ways that empower them to make informed choices about
their health and social care. Act as advocate for individual patients and
the client group.
To
work collaboratively, building relationships with other members of
multidisciplinary team and outside agencies to ensure individualised care is
provided in a timely and responsive way.
Provide
advice by telephone to patients, carers and other professionals, as required
from the Community Hub.
Give
and receive accurate reports and changes in patients condition, linking with
key clinical staff where required, to facilitate comprehensive and safe patient
care.
Maintain
accurate and contemporaneous records on System 1 and in all areas of clinical
practice.
Provide
a rapid response visit to a patients home when required, undertaking a full
clinical assessment of need, prioritisation and delivery of care.
Undertake joint clinical assessments with other professionals and liaise as
appropriate regarding ongoing care.
Use
of skilled decision making to determine appropriate prescribed medication to
alleviate symptoms.
Demonstrate
skills for assessing and interpreting any changes in a patient condition and
reporting/ escalating to appropriate professional, where required.
Be
aware of health and safety aspects of the service and adhere to lone worker
policy.
Promote
a safe environment for all patients, carers, visitors and staff in the
patients own home/inpatient unit as appropriate.
Effectively support the team
regarding Safeguarding and MCA, working closely with the leads within
Mountbatten
To
work collaboratively, building relationships alongside colleagues, to offer the
patient and family the skills of a dedicated palliative care service
encompassing symptom control, social, psychological and spiritual care.
To
participate in quality initiatives within the community.
Undertake
