Mountbatten Hospice supports people
across Hampshire who are facing death, dying and bereavement. Our staff are
professional, committed and passionate, and dedicated to providing outstanding
care and support to the communities we serve.
This role is based at Mountbatten
Hampshire Hospice, with a flexible working pattern to include day and night
shifts, including weekends and Bank Holidays.
Full-time: 37.5 hours a week

Main duties of the job
You will seek to provide a high standard of holistic, skilled, patient-centred, palliative care to all community patients,as part of Mountbatten Hampshire multidisciplinary community services team,providing nursing care under the direct clinical leadership of the Locality Lead/ Senior Nurse on Duty.
You will need to be an enthusiastic and motivated nurse, keen to develop both your practice and education, in line with the role requirements, and have been qualified for at least 2 years, having gained relevant experience.
You will need to hold a clean driving licence and have the use of a car with business insurance.

Job responsibilities
To be responsible for providing inspirational
and motivational clinical leadership as a senior staff nurse to a team on the
inpatient unit with the aim of delivering a high standard of specialist, patient-centred
palliative nursing care.
To work in conjunction with the Ward Manager/Deputy Ward Managers, and the
multi-disciplinary team and other Mountbatten teams, providing support to
patients and their carers.
To work closely with clinical colleagues to
establish and maintain high quality clinical care across all care settings, providing
consistent and effective communication with the team throughout your shift and
during handover.
To act as a role model,
providing training, supervision and support to staff, volunteers and students
and acting as an ambassador with external supporters and other key partners.
To
demonstrate specialist knowledge in palliative and end of life care.
To act as a resource of
clinical knowledge for Mountbattens staff and external agencies.
To work closely with
the coordination centre, and the housekeeping team, to manage admissions in a
timely and effective manner.
To lead on discharge
planning, facilitate discussions, and actively participate in discharge planning
meetings in liaison with other disciplines ensuring all relevant documentation
goes with them.
To advocate for
patients and other service users ensuring their views are known to the team
including in the case of advance care planning and occasions where offered care
is declined
To
effectively support the team regarding Safeguarding and MCA, working closely
with the leads within Mountbatten.
To have an active link
role supporting the team in an identified specific area (e.g. Tissue viability,
Infection Control, Falls, Nutrition and Hydration etc).
