to recruit an Occupational Therapist with these attributes who will, alongside
our specialist Community Nursing Team, visiting people in their home. Offering responsive, pro-active therapy
assessments and interventions to
individuals living with life limiting illnesses, you will enable more people to
remain in their preferred place of care and thrive in the community.
Main duties of the job
This new role will see the
successful Occupational Therapist candidate delivering skilled assessments and
therapeutic interventions to patients in their home, promoting a palliative
rehabilitation and enablement approach, embracing positive risk taking and
listening to what matters to the individual and their family. As part of the Rehabilitation and Enablement
Team, working closely with the Mountbatten Community Nursing Team, the
postholder will manage their own caseload, whilst being well supported in more
complex cases with opportunities for learning and joint working.
The Occupational Therapist will take
into account cultural and spiritual needs, and share their learning with the
wider team. As this is a funded post, they will also support with essential
data collection, audit and evaluation, to prove the impact of this novel role,
with the aim of attracting ongoing
funding and an extension of a longer-term or permanent contract.
This vacancy is
for 30 hours per week to be worked flexibly in accordance with the needs of the
service.

diverse palliative care needs in their own home. To provide, develop and
deliver individualised interventions, and to undertake these tasks as an
autonomous practitioner.

resource; providing consultative advice, education, and support to members of
the Hospice Multidisciplinary Team (MDT) and Health and Social Care teams.
Promote and support the development of this new and
