The community service provided by Mountbatten
Hampshire aims to provide care to patients and their families that is
supportive, professional and respectful of patients choices.
We are looking for a compassionate and dedicated Palliative Care Support Worker who - through excellent
assessment, being mindful of the quality of life and the meaning this has for
each individual - will offer intervention and care for patients with progressive
and life limiting illnesses enabling them to remain at home, in accordance with
their wishes.
This includes patients in
residential, nursing homes and other long stay settings for as long as
possible, according to their individual preferences.

and a full UK driving licence is essential for this role*
Salary Range: £25,147 - £27,596 per annum FTE (equivalent to Agenda for Change Band 4)
Main duties of the job
The role is responsible for supporting the
delivery of palliative and end of life care as part of Mountbatten Hampshires
community services team, providing nursing interventions and ongoing assessment
under the direct clinical leadership of the Advance Nurse Practitioners/Senior
Nurse on Duty reporting to the Locality Leads.
The purpose of the role is to work
independently, to assess and report the current patient status, support
patients and carers and initiate and deliver specific interventions as
required, in accordance with the supporting practice competencies.

- Promote a safe environment for all patients, carers, visitors and staff in the patients own home/inpatient unit as appropriate.
