Job summary

This role matters. We are looking for someone who is highly motivated, a good team player, enjoys a challenge and wants to make a difference to patient care, could it be you?

This role is crucial in supporting the work of the Hospice to meet the highest standards of governance and compliance, which is innate to our services, and you will need to support and work alongside all Hospice teams to achieve this at every level.

As a member of the Management Team you will be responsible for the development of a proactive and robust clinical quality, safety, infection prevention control and governance framework that enables St Johns to measure and continually improve the quality of its clinical services.

Excellent interpersonal, IT, analytical and organisational skills are crucial for this post. This is a role where you can use your drive, compassion and expertise to really make a difference to the care we provide to our patients and their loved ones.

This role is for 30 hours per week, and pay listed is based on these hours.

Main duties of the job

JOB PURPOSE:

This post will be responsible for the development of a proactive and robust clinical quality, safety, infection prevention control and governance framework that enables St Johns to measure and continually improve the quality of its clinical services.

The postholder will manage risk, incidents, audit and clinical effectiveness programmes in order to develop a high-quality governance Quality Improvement strategy across the Hospice, in discussion with the Medical Director and Director of Nursing and Quality.

The postholder will work closely with the Health and Safety Officer and the Head of Facilities to ensure the premises and equipment meet all regulatory and statutory requirements to keep patients, families, staff and volunteers safe and ensuring compliance is reported quarterly to the Board.

You will ensure all governance administrative systems and processes are in place and are operationally developed to enable effective assurance of monitoring, analysis and reporting on clinical and non-clinical risk management.

As a member of the Hospices Management Team contribute to the strategic development of Hospice services with an emphasis on governance and compliance. You will hold a working knowledge of palliative and end of life care and maintain a clinical presence with front line staff in all relevant services establishing a positive culture for transparency, learning and improvement.

St Johns exists to promote and provide skilled and compassionate palliative care of the highest quality. The Hospice provides care in a variety of settings, directly serving our local communities from Grasmere to Garstang and from Sedbergh to Morecambe. The Hospice is rated Good by CQC.

Hospice care at St Johns is provided free of charge. The NHS contributes around 25% of the Hospices annual running costs, the remaining being obtained via the generosity of the public.

What we do:

St Johns Hospice provides Specialist Palliative Care services and support to those affected by life-shortening illnesses.

Our vision is to put local patients and families at the heart of everything we do to ensure the right care, in the right place, at the right time on the journey towards the end of life.

To provide these services, the Hospice employs around 220 staff and is well supported by our dedicated volunteers.

St John's is governed by a Trustee Board that takes overall responsibility for its work and is managed by a Senior Management Team.

The Hospice is a Charitable Incorporated Organisation and the responsibility for its governance and management falls on our Board of Trustees.

MAIN DUTIES

  • Ensure the Hospice meets CQC compliance with the Assessment Framework (and relevant future assessment frameworks) and CQC inspection regimes across all services, departments, and processes, aiming to raise the Hospices inspection rate back to outstanding.
  • Develop and implement as Hospice Quality Improvement strategy with input from relevant members of SMT. This will be aligned to internal and external reporting and assurance requirements and follow the principles of PSIRF to enable the organisation to measure and continuously improve the quality of the service it provides.
  • Organise and chair the quarterly Clinical Operations meeting. Liaise with the Director of Nursing & Quality before sending meeting invitations, agenda and supporting information to meeting attendees.
  • Monitor and report on clinical risk and incidents, including risk assessments, incidents and the directorate risk register.
  • Ensure responses are proportional and in keeping with the work of the Hospice, balancing Safety 1 and Safety 2.
  • Analyse qualitative and quantitative data from several sources in governance and clinical areas to ensure relevant actions and recommendations reach beyond just the results of the quality and improvement work.
  • Contribute to strategy and work on ad hoc projects as appropriate.
  • Submit data and evidence to relevant external agencies, e.g. Hospice UK, CQC
  • Ensure all that staff are engaged effectively and efficiently in governance processes by building relationships with clinical staff and departments.
  • Monitor complaints and compliments across all services, identifying themes and trends which, by addressing, may change practice and improve our care.
  • Develop a robust clinical governance Quality Improvement Plan providing updates to the Director of Nursing and Quality on a quarterly basis ensuring the Hospice meets all quality assurance standards.
  • Share lessons learned in adaptable ways to all departments, ensuring changes to practice are mirrored in policies, training and competences as required
  • Work with the Head of Facilities and Health & Safety Officer to ensure the maintenance of the building and equipment meets all regulatory requirements. Ensure these are recorded for CQC and HSE compliance purposes.
  • Support St Johns as a learning organisation, sharing lessons learned in adaptable ways to all departments.
  • Work in partnership with The VOICE Co-ordinator to implement the patient feedback protocol i.e. the patient and family survey process
  • Consistently review how data is presented at meetings, revising reports as necessary, to ensure recognition of trends and themes.
  • Support department managers in their responsibilities for compliance and Board assurance.
  • Manage what is stored in the CQC evidence database, ensuring evidence accurately showcases the work of the Hospice.

Infection Prevention and Control

  • Lead the infection prevention and control work across the Hospice to meet CQC requirements, meeting with the IP&C link nurses and Head of Facilities quarterly ensuring all relevant audits give full assurance to the Board.

  • Apply PSIRF to all elements of risk management in patient care, updating the PSIRP annually

  • Support clinical data requests and reports, working with the IT department, to ensure data is consistent and accurate for the Board and for ongoing commissioning.

  • Educate staff in audit, ensuring accurate completion and reporting.
  • Analyse clinical data from electronic patient records (EMIS) across all services, highlighting areas of unmet or increasing need such as specific disease groups, faith groups and hard to reach patient groups, producing reports on how the Hospice can support these going forward.

  • Understand how information governance applies in patient and staff records, ensuring policies and procedures are adhered to at all times.
  • Advise on organisational compliance with confidentiality, clinical information governance and data protection in patient care.

Risk and incident management

  • Review trends in clinical incidents and complaints, ensuring that After Action Reviews are completed (PSI if needed) and change implemented when required to improve quality, safety and patient experience.
  • Monitor complaints regarding the clinical service and ensure a response to patient and care satisfaction surveys address any negative feedback when necessary.
