Job summary

An exciting opportunity has arisen for a highly motivated and experienced Advanced Paramedic Practitioner (APP) to join our expanding team.

This innovative role is focused on transforming the current paramedic workforce, enabling clinicians to develop from the management of general minor ailments, to confidently managing same-day care and frailty presentations.

The post holder will work 4 days clinical for the Minor Ailments Service, with 1 day protected to focus on the clinical development of the team.

The post holder will be instrumental in building capacity, developing skills, and embedding high-quality frailty care pathways across three diverse and dynamic Primary Care Networks. You will play a key role in shaping our integrated approach to urgent, proactive and personalised care, working closely with GPs, nurses, allied health professionals and wider community teams.

Main duties of the job

Provide autonomous, high-level assessment, diagnosis, and management of patients with undifferentiated or complex needs, including those living with frailty.

Deliver safe and effective same-day care through a minor ailments service and home visits.

Lead by example in the delivery of high-quality clinical care across PCN sites.

About us

Quay Healthcare is a non-profit company owned by the GP practices in Warrington.The PCN is headed by our committed Clinical Directors, Strategic Manager, Practice Managers and GP members.

Quay Healthcare supports three Primary Care Networks (PCNs), based in Warrington, working collaboratively with East Warrington PCN, Central East Warrington PCN and SWAN (South Warrington PCN). There are 13 practices across the 3 PCNs.

Our vision is to provide innovative staff and services to patients and the PCNs. We have successfully employed clinical pharmacists, pharmacy technicians, first contact physiotherapists, mental health practitioners, care coordinators, and first contact practitioners.

We are looking for an exceptional Advanced Paramedic Practitioner to upskill the existing Paramedic team of six. The successful candidate will support the development of all Paramedics, across the three PCNs. You will work alongside the Medical Director, providing updates to each PCN on the development of the teams progress.

  • Experience of leading complex projects including service redesign, in challenging environments, including, interpretation and the comparison of a range of options.
  • Effective change management and process redesign experience.

  • Ability to engage effectively with clinicians, senior managers, Board members, staff, other key agencies, and the public.
  • Ability to communicate ideas clearly and concisely within and outside the organisation; ability to describe and articulate proposed service and patient benefits as part of gaining support for positive change.

Desirable

  • Knowledge of frailty assessment tools (e.g. Rockwood CFS, eFI) and geriatric care approaches.
  • Knowledge of PCN priorities
  • Familiar with digital systems and population health tools
