Job summary

The post holder will work alongside the PCNs Strategic Manager, the Clinical Director and the wider team to achieve success in line with the PCN Network Contract DES and other local priorities. You will provide day to day operational support to the PCN, and lead on the coordination of the vaccination service, along with other PCN Clinical Projects.

About us

In addition to the PCN our Practices are also part of Quay Healthcare, which is a non-profit company owned by GP Practices in Warrington. Quay Healthcare provide business and practical support to the PCNs and deliver additional healthcare services to our patients. The PCNs contract GP provider services through Quay Primary Healthcare CIC who are responsible for providing a number of services on behalf of the PCN; one of these services includes the management of our PCN ARRS staff, therefore this means that you will work alongside members of the PCN and Practice staff on a daily basis, with Quay Healthcare as the lead employer.

Job responsibilities

Vaccination Programme (COVID and FLU)

Whilst working for the vaccination programme, duties may include:

  • Amalgamating and managing the networks vaccination plans
  • Ordering of vaccine and consumables by the required deadlines
  • Inputting vaccine data
  • Providing data and information to the ICB regarding the networks vaccination efforts where required
  • Building strong working relationships with the networks practices, answering vaccination queries and supporting practice roll out of the various phases of the programme.
  • Arranging locum staff where required
  • Supporting practices with communications to patients where required
  • Arrange appointments with patients
  • Providing admin support on site.
  • Represent the PCN at relevant meetings

Day to Day PCN Coordination

  • To work with and support the PCNs Clinical Director, Strategic Manager, PCN members and their Teams to ensure effective delivery of the PCN DES and IIF requirements
  • Responsible for assisting with day-to-day operational delivery
  • Support equity of service provision across the Networks 5 core practices
  • Trouble shooting day to day and responding to any queries.
  • Coordination of the PCN Board Meetings, and minute taking
  • develop and maintain effective communication both with the ARRS staff and with all partner practices.
  • To provide support with project implementation and communications for new clinical services.
  • Support the PCN to develop/ maintain a patient voice forum, patient facing comms and social media presence for the PCN
  • Organise meetings or events related to PCN activity.
  • Provide secretarial support to the PCN Executive Board and Clinical Leads Meetings
  • Where required, take a lead role in project delivery. This could range from individual PCN projects to supporting clinicians in the development of new pathways of care.
  • Help develop joint working and relationships across networks and its stakeholders.
  • Communicate information and issues, including briefings and reports.
  • Maintain excellent relationships across all core member Practices.
  • To produce, review and circulate documents required for meetings.
  • Minute meetings. Collating action points and circulating.
  • Supporting PCN members with all other reasonable administration requests.
  • Supporting and collating evaluation of PCN services to drive further improvements.
  • Support the financial delivery of PCN activities.

  • Keep abreast of national and local guidance on PCNs

  • take responsibility for maintaining a record of own personal and professional development

  • Liaise with other stakeholders as needed for the collective benefit of patients

  • Utilise population health intelligence to proactively identify and work with a cohort of patients to deliver personalised care
  • Support patients to utilise decision aids in preparation for a shared decision-making conversation. holistically bring together all of a persons identified care and support needs, and explore options to meet these within a single personalised care and support plan (PCSP), in line with PCSP best practice, based on what matters to the person
  • Support the Network with coordination of Clinical projects in line with the PCNS priorities.
  • Help people to manage their needs through answering queries, making and managing appointments, and ensuring that people have good quality written or verbal information to help them make choices about their care
  • Assist people to access self-management education courses, peer support or interventions that support them in their health and wellbeing and increase their activation level.
  • Provide coordination and navigation for people and their carers across health and care services, working closely with social prescribing link workers, health and wellbeing coaches, and other primary care professionals; and support the coordination and delivery of MDTs
