Advanced Practitioner - Clinical Pharmacist

Job summary

To lead and co-ordinate the professional clinical and pharmaceutical input into the safe and cost-effective management and development of the utilisation of medication in conjunction with Practice requirements and PAN Mersey guidelines, ensuring safe and secure use and minimising the risk of inappropriate use.

To provide professional, managerial, and clinical leadership to a team of pharmacy staff involved in the delivery of clinical pharmacy and medicines management service within primary care ensuring safe effective and evidence based pharmaceutical care.

Assess and triage patients, including same day triage, and as appropriate provide definitive treatment (including prescribing medications following policy, patient group directives, NICE (national) and local clinical guidelines and local care pathways) or make necessary referrals to other members of the Primary Care team.

Manage undifferentiated undiagnosed condition and identify red flags and underlying serious pathology and take appropriate action

Use complex decision making to inform the diagnosis, investigation, complete management of episodes of care within a broad scope of practice.

Actively take a personalised care approach and population centred care approach to enable shared decision making with the presenting person.

Main duties of the job

Document general and highly specialist pharmaceutical care issues and care planning in patients case notes, verbally informing medical/nursing staff of urgent/important actions to be taken.

Participate in the implementation, communication and delivery of the clinical governance and medicines management objectives of Quay Healthcare. Take the lead in implementing such objectives in own area of responsibility (e.g. Implementing and monitoring adherence to new product introduction guidance, NICE guidance and safe, cost effective, appropriate prescribing initiatives). Communication / consultation with Medicines Management Team to ensure safe prescribing.

Provide additional specialist information and provide support/clinical supervision to junior pharmacists with respect to these activities in designated specialism.

Overcome barriers to communication in order to provide proactive and reactive pharmaceutical information and advice to prescribers, other healthcare professionals and patients.

Lead on the development and implementation of new Pharmacy and Practice-wide guidelines and policies and the review of existing ones.

Comply and ensure that other staff members comply with the Medicines Act, the GPhC standards for pharmacy professionals and other relevant legislation.

Support Senior Pharmacy Management with other activities associated with CQC requirements, Governance, Risk Management and more specifically Medicines Management

Job description

1.6 Provide additional specialist information and provide support/clinical supervision to junior pharmacists with respect to these activities in designated specialism.

1.7 Overcome barriers to communication in order to provide proactive and reactive pharmaceutical information and advice to prescribers, other healthcare professionals and patients taking into consideration their knowledge, skills, level of understanding and disabilities.

1.8 Use tact and diplomacy when communicating sensitive medicines information to patients who, for example, have not yet been informed of their diagnosis or where the medication has been started in advance of a confirmed diagnosis.

1.9 Provide health promotion advice to patients or carers.

1.10 Communicate effectively with other Pharmacy colleagues to convey information that ensures continuity of pharmaceutical care for individual patients and the availability of adequate supply of medication to meet patients needs.

1.11 Communicate with prescribers and/or other healthcare professionals & the Medical Director to determine the course of action to be taken when a medication incident or near miss occurs and when necessary, discuss the incident with a patient or their relatives in order to provide advice, guidance, or reassurance.

1.12 Comply with Quay Healthcares Policy on documenting critical incidents and near misses and assist with root cause analysis and the implementation of changes in practice to reduce risk.

1.13 Attend staff meetings or read the minutes of staff meetings and put into effect any actions as appropriate.

1.14 Personal & People Development. Provide clinical supervision, training and support for clinical pharmacists and qualified clinical pharmacy technicians.

1.15 Be responsible for clinical pharmacy/pharmacy technicians education within primary care. Encourage and provide support (mentorship / education & training) to enable other staff to undertake continuing professional development.

1.16 Identify the need for and provide relevant education / support to patients and/or carers.

1.17 Undertake continuing professional development as per national guidelines from the General Pharmaceutical Council. Maintain and update own competencies and with the Principal Pharmacist(s), identify own training needs and courses to develop within designated specialism.

1.18 Participate in the Quay Healthcares performance review process and, having received the necessary training, undertake personal development reviews and produce personal development plans for other staff members as required.

1.19 Work towards objectives agreed at Personal Development Review and develop own Personal Development plan with the manager.

1.20 Attend and contribute to shared workplace meetings or read the minutes of such meetings.

2. Quality

2.1 Lead on the development and implementation of new Pharmacy and Practice-wide guidelines and policies and the review of existing ones.

2.2 Ensure that the quality and accuracy of all work is maintained despite frequent, unpredictable, interruptions.

2.3 Within the immediate work area, ensure all actions and activities of oneself and other pharmacy staff members are performed and checked in accordance with current medicines legislation and with guidelines issued by the Health and Safety Executive, Medicines Inspectorate, or other appropriate body.

2.4 Comply and ensure that other staff members comply with the Medicines Act, the GPhC standards for pharmacy professionals and other relevant legislation.

3. Lead Pharmacist Work / Medicines Management

3.1 Support Senior Pharmacy Management with other activities associated with CQC requirements, Governance, Risk Management and more specifically Medicines Management

3.2 Undertake horizon scanning for new products about to enter the market.

3.3 Lead on the production of written documentation to support improvements in the use of medicines within Primary Care. Liaise and agree content with pharmacy staff representing primary care interests and submit documentation to Quay Healthcare.

3.4 Lead on the implementation of local and national guidelines as required.

3.5 To undertake the full range of duties of a line manager for any assigned pharmacists including appraisals, development revies, performance management and involvement in the recruitment process.

4.1 Undertake audits and research work for Quay Healthcare in order to develop and improve services and to ensure compliance with local and national guidelines.
