Job summary
This service has been developed in partnership with Vale of Glamorgan County Council and the Housing Solutions Team. To improve the lives of people with mental health who require support because they are homeless or at risk of being homeless. By providing one-to-one support to equip individuals with the skills, knowledge and coping strategies to ensure their own well-being.
Under the direction of the Senior Support Worker and Service Manager, the post holder will work as part of the overall Adferiad team in the Vale of Glamorgan area. The support is to be delivered in line with Adferiads principles of empowerment, self-management and recovery, and to ensure service compliance with internal and external quality standards, and regulatory requirements.
The Link Worker:
Will be based at Vale of Glamorgan Councils Housing Solutions Team, Barry and/or Hot Desk at other Service Locations.
Will provide to allocated referrals, Outreach work to support homeless/potentially homeless people who have mental health needs.
This role is for a person who can work closely with Housing Solutions to provide a link with specialist Mental Health services and pull services together to carry out complex case work.
The service will operate Monday to Friday 9 a.m. to 5 p.m. BUT will have the flexibility to work some evenings, as required.
Main duties of the job
- Receiving and dealing with referrals from the Housing Solutions Team.
Processing of approved new referrals following procedures for completing new assessments, risk assessments and creating individual person-centred service delivery plans.
The Link Worker will be expected to identify the causes and actively find solutions to peoples homelessness in a time limited period in accordance with the persons housing and related support needs.
The Link Worker is to provide intensive support to each person in order to move on and / or signpost the person to the most appropriate services within a 6-week period.
This service includes supporting individuals who are at Crisis point and urgent risk of homelessness.
The Adferiad Link Worker is to provide a variety of person centred, tailored interventions that support individuals with complex needs to find sustainable accommodation and develop their independent living skills.
Manage a client caseload, and complete individual needs assessments, risk assessments, support plans and offer practical advice.
Follow procedures for conducting individuals monthly reviews as and when required due to changes in individuals needs or circumstances, ensuring risk assessments and support delivery plans are kept up to date and are relevant.
