Clinical Specialist Non-Medical Prescriber

Job summary

This is an exciting opportunity to join an experienced and passionate clinical team, who are dedicated in delivering excellent palliative care, to patients in the community. As an individual, you will need to possess excellent communication skills, have extensive knowledge in palliative care, champion a culture of compassion and be able to work under pressure.

In this role you will be supporting patients, who have a terminal diagnosis and need support with the challenges they face, so they can remain in their home. The role does require a positive can-do attitude, where hurdles are not seen as barriers but as challenges to overcome. You will need an ability to work as a team, as well as autonomously and be prepared to travel to patients homes.

As a prescriber, you will need to feel confident with making prescribing decisions which align to palliative guidelines, while also demonstrating excellent ability to rationalise choices made. In addition, as a senior part of the team, you will need to offer education, support and leadership to your colleagues who are developing in their roles. The role also does entail a need to be flexible to the demand of the day and to contribute to team clinical meetings, where additional perspectives are welcomed.

Main duties of the job

To work as part of our specialist, highly skilled and dedicated Hospice at Home team, making sure our patients feel comfortable and at ease in familiar surroundings

To act as a resource to patients in East Berkshire and South Bucks who are on the End-of-Life Register by offering assessment, complex symptom control, emotional support, advanced care planning, advice and treatment

To act as a resource to fellow professional colleagues across the locality of East Berkshire and function autonomously in providing excellent end-of-life care advice

Specific Role Responsibilities:

Use specialist skills and high-level decision making, including the evaluation of research and evidence, in the assessment and management of complex symptoms in Nurse led clinics or patients homes

Use advanced communication skills to negotiate, influence and challenge practice in all settings

Assess and plan effective end of life care interventions, communicating these planned interventions clearly and concisely both in verbal and written format to other Health Care Professionals

To instigate appropriate treatment following assessment (including prescribing medication as an independent non-medical prescriber)

Monitor and evaluate the outcome of planned care

Work collaboratively with hospice-wide services to ensure all patients have access to RN/Paramedic assessment when required. Use high level communication skills to elicit concerns, establish and maintain a trusting and therapeutic relationship with patients and their family/carers facilitating the sharing of sensitive and emotive information relating to prognosis and disease, death and dying

Promote effective communication, coordination of care and appropriate referrals to other services

Proactively co-ordinate and manage a caseload

Respond to urgent calls for symptom management to prevent unnecessary hospital admissions

Attend GSF meetings to discuss patients on caseload

Attend Community MDTs to facilitate learning and ensure best practice

Participate in a 7-day a week service across the locality

Provide mentorship and facilitation to other professionals with less experience in end-of-life care

Prescribe, where appropriate and competent, for palliative care symptoms in line with personalised intention to prescribe documents, Thames Hospice and local prescribing policies

Attend monthly Thames Hospice NMP reflective practice forum

Support junior staff in the Hospice at Home team and each other when required

Provide teaching and training to patients/carers and other groups at appropriate levels

Participate in external educational activities in collaboration with the Thames Hospice Education Department

Provide educational mentorship and facilitation to new team members and students

Ensure that the team meet contractual and organisational KPIs

Participate in audit's of the service, to identify if improvements and/or changes have enabled an improved patient and care experience

Report accidents, incidents and complaints in area of responsibility as per procedure, participating in investigations as required

Ensure staff understand and work to the processes for lone working, role boundaries and patient confidentiality
