Job summary

To support patients with a palliative or end of life prognosis and their families/carers towards achieving their preferred place of care in their own home

To work under the supervision of the CNS to develop knowledge and expertise in Palliative Care whilst providing a timely and seamless service.

Managing corporate caseload of patients with other SSNs, telephoning to support and visiting as needed, supporting Duty Clinical Nurse Specialist (CNS) with incoming calls on a rotational basis.

Please note, you must be a registered nurse or be a qualified paramedicto apply, but experience of palliative or end of life care isn't essential.

Main duties of the job

We are currently looking forexperienced Nurses to join our busy Hospice at HomeTeam as a Senior Staff Nurse to assist in providing care for patients requiring end of life care in their own homes.

In this role, you will report to a dedicated Clinical Specialist and work across a locality to provide individual care, including symptom control with the support of a Health Care Assistant.

You will need to have experience as a registered nurse or be a qualified paramedic, but experience ofcommunity working and palliative care is not essential. You should possess good communication skills, excellent care values and a strong sense of team spirit.There is the opportunity progress your career to Senior Staff Nurse subject to achieving competencies.

Job responsibilities

To ensure that the PCRT delivers an excellent standard of care to patients and their families

To be aware of own accountability and limitations and take responsibility for caseload/time management with the support of the CNSs

To work autonomously to deliver individualised care based on need, using assessment and reassessment of nursing and symptom control needs as appropriate

To provide psychological support to patients and their families/carers

To communicate fully and effectively with patients, families and professional colleagues

To identify and appropriately manage barriers to effective communication, acting as patients advocate if needed

To work collaboratively with the wider Multidisciplinary Team (MDT), GPs, District Nurses and clinical staff within the Acute Trusts, to ensure a co-ordinated and defined management plan for each patient; to include review and discharge as appropriate

To perform VOED (training will be provided) and last offices if required

To participate in training of Student Nurses, Health Care Assistants and Paramedic students

To maintain accurate healthcare records in line with Thames Hospice organisational policy and own professional accountability

To develop and maintain effective partnership working with the IPU Medical and Nursing Teams

To achieve service specific targets

To participate in audits of the service to identify if working practices and changes have enabled an improved patient/carer experience

To report accidents, incidents and complaints as per procedure and Policy, participating in investigations if required July 2023

To understand and comply with processes for lone working, role boundaries and patient confidentiality

To maintain personal and professional development including attendance at Thames Hospice mandatory training
