Youll be part of a team of registered nurses and carers, with consultant oversight, providing day to day leadership, management and continuous improvement of the delivery and services of the virtual ward.
Working with the Education Team and Nurse Consultant, youll educate and support our colleagues to achieve their full potential, and to feel valued, competent and confident in their roles.

Main duties of the job
Working collaboratively you will be an integral part of an amazing multi-disciplinary team, working with doctors, clinical and pastoral care teams, therapists, physios and great support teams. Together youll ensure a high standard of provision of care for patients requiring palliative and end of life care.
Youll be part of a team of registered nurses and carers, with consultant oversight, providing day to day leadership, management and continuous improvement of the delivery and services of the virtual ward.

Salary: £33,674 - £45,317 (based on full time working)*
* Work on a Saturday: time and one third - 1.33
Work on a Sunday or Bank Holiday: time and two thirds - 1.66
Service Delivery
1. To lead the Virtual ward when on shift, acting as a role model and provide support and advice to junior staff on the delivery of care including symptom management
2. To offer timely and individual care based on need, using referral criteria, regular patient assessment, re-assessment and discharge criteria as appropriate
3. To communicate fully and effectively with patients, their families and professional colleagues to ensure a safe and needs led care pathway
4. To work as a member of the multi-disciplinary team and promote multi-disciplinary working at all times. To attend MDT meetings as required
5. To maintain accurate healthcare records, written and computer based, in line with Thames Hospice organisational policy and professional requirements
6. To develop communication links with external care agencies, GPs and community teams and senior clinical staff within acute trusts to facilitate best working practice and service provision
7. To participate in training, development and mentorship of staff, student nurses and external stakeholders as required
8. In conjunction with the Practice Educator, develop and monitor competencies for Registered Nurses, by regularly working with them as part of a shift
9. To rota staff on virtual ward and ensure all patients have safe and adequate oversight, delivering a high standard of care.
10. To visit each patient daily and add in care or refer accordingly.
11. To maximise on bed occupancy (5 patients at home)
12. To work with Head of Thames care @ home and Coordinators on the packages of care necessary.
13. To participate in audits of the service, and identify if improvements and/or changes are needed to enhance patient care and experience
14. Ensure compliance with Health and Safety procedures and to report accidents, incidents and complaints in area of responsibility as per procedure, participating in investigations as required
15. To ensure staff understand and work to the processes for role boundaries and patient confidentiality
16. To work with the Consultant / specialty doctor to provide and meet all care required.
17. Responsible for the administration of prescribed medicines for symptom management as per authorisation and Palliative Care Guidelines
