Are you are an enthusiastic & caring nurse with excellent communication skills looking for an opportunity to give your best & become part of an outstanding, friendly team? We are seeking nurses with a passion for palliative care, highly motivated & forward thinking to work within the existing caring & dynamic team of nurses. These positions will have occasional rotation to nights within the Inpatient Unit (IPU).

You will be expected to have an understanding of the importance of multi-professional team working that are vital to effective team working & practice them at all times & to participate in change & development of practice. Participate in organising the individual nursing team in the absence of the Band 5B/Ward Sister/Charge Nurse & to co-ordinate the IPU unit in their absence. Also to participate in ensuring adequate staffing of the IPU. Maintain effective communication networks with all departments within the hospice to maintain a seamless delivery of specialist palliative care services.

You will take responsibility for your professional development, which will be monitored as part of the annual appraisal review. Maintain current NMC registration & adhere to the NMC Code of Conduct at all times.

Full/part time working hours considered, salary pro rata for part time hours. Enhancements paid for shifts worked overnight, weekends & on Bank Holidays.

Main duties of the job

Provide appropriate physical, emotional and spiritual care in agreement with the patient and their family. Consult and liaise with both the nursing and multidisciplinary teams to ensure continuity of holistic patient care. Use evidence based practice when planning patient care. Participate in the hospice quality assurance programme. Communicate effectively and appropriately with patients and carers. Liaise with other professionals involved in their care. Recognise and report changes in the patients condition and respond appropriately to maintain effective symptom control. Implement nursing procedures correctly. Ensure that patient documentation reflects a holistic account of patient interventions. Maintain clinical records and associated documentation written and electronic in accordance with SFH policies. Accurately document care given within NMC guidelines. Participate in the annual hospice UK and clinical audit programmes to evaluate areas of clinical practice as requested. Be a reflective practitioner. Have an identified area of special interest of clinical practice.

Communicate effectively with patients, relatives and other visitors. Participate in discussion, ensure effective communication with the clinical team when planning and evaluating patient care needs. Participate in multidisciplinary team meetings to ensure high quality holistic care for patients and their families.To effectively use IT systems to ensure patients records are kept updated on ICARE & CMC records.

We support 5 Clinical Commissioning Groups & have a robust governance structure in place. We provide inpatient clinical services within a modern, personal & friendly 18-bedded unit, as well as Specialist Community Crisis Support Service, Hospice at Home Service & Specialist Day Therapy Service. We also have the hospice Pepperell Education Centre that has an excellent reputation for delivering high quality education programmes and study days.
