Job summary
Full or Part time considered, £34,185 to £40,491 a year full time, pro rata salary for part time hours, plus enhancements for unsocial hours worked.
To provide appropriate palliative care advice and support to patients and their carers through telephone Specialist Advice Line, 09.00 17.00 Monday to Friday.
To act as a resource for the Specialist Community & Crisis Support Service (SCCS) and ensure robust links across Saint Francis Hospice and external partners.
Responsible for creating relationships with other teams in the community to provide best support at home including DN teams, GPs, Acute Hospital Trusts, Care Providers & LAS/East of England
Be responsible for own development to being an autonomous expert practitioner in specialist palliative care.
Able to manage complex distressing/highly emotional situations on a regular, potentially daily basis
To take part in audits and other service assessment work as evaluation of the community service processes.
To act as an ambassador for the service and the Saint Francis Hospice at all times including representing with occasional fundraising and PR activities as requested.
To line manage and given supervision to the 5b Advice Line Senior Staff nurse post.
Main duties of the job
A dynamic, enthusiastic, self-motivated, experienced qualified nurse or HCPC with a clinical palliative care knowledge base is required, to join our well-established specialist community & crisis service (SCCS) to support our advice line Monday to Sunday 9-5pm on site. This post will provide varied senior experience in a multidisciplinary working environment. This role is for telephone support only. No patient facing contact but the opportunity to work alongside other clinicians on occasions.
The SCCS works within the boundaries of Barking & Dagenham, Havering, and Brentwood providing out of hours advisory telephone support to include Redbridge. Working collaboratively with other internal and external health care professionals.
Do you have excellent communication, assessment and prioritisation skills, and understands the needs of those receiving palliative care /end of life care, a working knowledge of evidence based practice and the ability to develop a good knowledge base of current initiatives and developments in palliative care. Any community experience with lone working involved would be valuable as well as sharingyour palliative care skills and expertise with us.
