Job summary
We are looking for a Palliative Care Co-Ordinator.
Can you, once trained, provide advice and support for patients and their families or carers? Our care co-ordinators will help patients navigate health and social care services and signpost to partner and voluntary organisations.
Main duties of the job
Work out of our care co-ordination centre, manning and triaging phone calls and web enquires. Act as a single point of access for healthcare professional and patients and families. To signpost and provide information on services as required.
