About us

Sue Ryder St Johns Hospice is in Moggerhanger in Bedfordshire. We provide expert palliative and end-of-life care in the hospice and the community. We are here to help you and your loved ones, with everything from pain and symptom management to emotional and practical help.

Main duties of the job

About the role

This is an exciting opportunity for an experienced Nurse who wants to work in a supportive environment where care of the patients comes first. This role is based within our friendly and supportive Community Team and involves visiting people in their own homes to assess, plan care and provide support to people when they need it most.

The successful applicants will play an active role within our multidisciplinary team including Specialty Doctors, therapy staff, volunteers, other nurses and other vital members of the MDT. Working with our patients and their families in the community requires a holistic approach to ensure the best possible outcomes Whether in the last months, weeks or days of life, or living with grief, we help people live the best life they possibly can.

Minimum Essential Criteria

NMC registered and Experience delivering or managing community-based clinical care

Skilled in implementing care and access adjustments

Strong multidisciplinary communication skills

Ability to travel independently

Understanding of equality and individualised care planning

Desirable Criteria

Experience in case management or community coordination

Non-medical prescribing qualification or interest in developing

About us

About you

Our ideal candidate will have experience of working in Palliative and End of Life Care. This experience may come from a hospice or alternatively, for example, from a high dependency unit, oncology or community service. They will be comfortable with leading junior staff and be able to offer support and advice when needed.

NMC Registered Nurse with post-registration UK experience in the clinical field

UK Driving License and access to a vehicle essential

Completion of NMC recognised accredited mentor training or willing to undertake

Evidence of continued professional development

Proven ability to build, lead and influence an effective team in a clinical setting

Knowledge of current legislation within the independent care sector

Knowledge of best clinical practice for client group

Demonstrable experience of practising in a self-directed and highly motivated way

Sue Ryder provides a thorough induction with supernumerary time to settle into the role and full training/competency assessment will be provided.

There is plenty of scope to further develop your own skills in specialist palliative care, and planned career progression through our education programme and development framework is encouraged.
