Leckhampton Court is a Sue Ryder Hospice and is the only specialist palliative care unit in Gloucestershire.
We support patients with life-limiting conditions to live the fullest life they can in both the inpatient unit, in their own homes and in our innovative community Hub.

Main duties of the job
The Community Hub
The Community Hub at Leckhampton Court Hospice offers day services to adults with a life limiting diagnosis. We support patients, their carers and loved ones, with a focus on wellbeing, quality of life and rehabilitation.
We offer a growing range of programmes and group sessions, as well as individual appointments in our outpatient clinics. We work as a multidisciplinary team of palliative care specialists, with the patient's needs at the centre of everything we do.
The Therapies Team lead programmes such as fatigue and breathlessness management and exercise groups. We will be launching a living well programme in early spring. Volunteers also offer social groups and creative arts.

The position is based within the Community Hub, answering to the Community Services Manager this post provides a great development opportunity for someone who is interested in furthering their career in healthcare or specialising in palliative care.
The role will be primarily based in the Community Hub. You will be assessing individuals needs, to create bespoke care plans, with the support of the multidisciplinary team. You will monitor patients and evaluate care as part of the role.
Working with the Community Services Manager, you will contribute to the development of new programmes and facilitate group sessions with the support of the multidisciplinary team.
There is a range of opportunities to develop academically and professionally for the successful candidate.
