Job responsibilities
Undertake work in line with PCN directed
priorities.
Act as the first port of call for
patients in relation to their care.
Ensure regular and consistent
communication with the referrer regarding patient progress and any
complications or guidance.
Administer tasks and ensure referrals are
completed and care delivered by keeping in regular telephone contact.
Develop and maintain engagement with
practice staff and encourage best practice.
Provide coordination and navigation for patients
across health and care services, alongside working closely with other primary
care roles.
Support the coordination and delivery of
care home MDTs within the PCN, supporting delivery of the enhanced health in
care homes model.
Support the PCN to collate patient and
carer feedback on their experiences.
Support national screening programmes.
Support PCN and patient liaison with other
agencies to coordinate care for patients.
Where required, proactively identify and
work with a cohort of people to support their personalised care requirements,
using the available decision support aids.
Refer to patients to PCN social
prescribing link workers where a patient is identified as potentially
benefitting from this service.
Help people to manage their needs,
answering their queries and support them to make appointments.
Support the delivery of personalised
care in care homes/LD homes through collaborative working between health,
social care, voluntary, community and social enterprise sector and care home
partners.
Ensure that people have good quality
information to help them make choices about their care.
Signpost people to access
self-management education courses, peer support or interventions that support
them in their health and wellbeing.
In the course of seeking treatment,
patients entrust us with, or allow us to gather, sensitive information in
relation to their health and other matters. They do so in confidence and have
the right to expect that staff will respect their privacy and act
appropriately.
In the performance of the duties outline
in this job description the post-holder may have access to confidential
information relating to patients, their carers, practice, or staff information.
All such information from any source is to be regarded as strictly
confidential.
Information relating to patients,
carers, colleagues, or member practices may only be divulged to authorised
person in accordance with PGPA policies and procedures relating to
confidentiality and the protection of personal and sensitive data, or under the
guidance of your manager.
To
maintain up to date knowledge of legislation, national and local policies and
issues in relation to the specific client group, mental health and community
care delivery.
To
comply with the Professional Codes of Conduct and to be aware of changes in
these. To maintain up to date knowledge of all relevant legislation and local
policies and procedures implementing this.
To ensure that all duties are
carried out to the highest standard and in accordance with currently quality
initiatives within the work area.
To comply with all relevant organisations
policies, procedures and guidelines, including those relating to Equal
Opportunities, Health and Safety and Confidentiality of Information and to be
aware of any changes in these.
To comply at all times with
the PGPAs Information Governance related policies. Staff are required to
respect the confidentiality of information about staff, patients and PGPA
business and in particular the confidentiality and security of personal
identifiable information in line with the Data Protection Act. All staff is
responsible for ensuring that any data created by them is timely,
comprehensive, accurate, and fit for the purposes for which it is intended.

PGPA

and permit us to collect and retain sensitive information relating to their
health and other matters pertaining to their care. they do so in confidence and
have a right to expect that all staff will respect their privacy and maintain
confidentiality at all times. It is essential that, if the legal requirements
are to be met and the trust of our patients is to be retained, all staff
protect patient information and provide a confidential service.
Quality
and Continuous improvement (CI)
To
preserve and improve the quality of our output, all personnel are required to
think not only of what they do, but how they achieve it. By continually
re-examining our processes, we will be able to develop and improve the overall
effectiveness of the way we work. The responsibility for this rests with
everyone working within the practice to look for opportunities to improve
quality and share good practice.
This
practice continually strives to improve work processes which deliver health
care with improved results across all areas of our service provision. We
promote a culture of continuous improvement where everyone counts, and staff
are permitted to make suggestions and contributions to improve our service
delivery and to enhance patient care.
Induction
training
On
arrival at the practice, all personnel are to complete a practice induction
programme. This is managed by the Practice Manager.
Learning
and development
The
effective use of training and development is fundamental in ensuring that all
staff are equipped with the appropriate skills, knowledge, attitude and
competences to perform their role. All staff will be required to partake and
complete mandatory training as directed by the training coordinator, as well as
participating in the practice training programmes. Staff will also be permitted
(subject to approval) to undertake external training courses which will enhance
their knowledge and skills, progress their career and ultimately enable them to
improve processes and service delivery.
Staff
will be required to participate in regular supervision in accordance with good
practice guidelines and policy and take part in the organisations annual appraisal
process.
Collaborative
working
All
staff are to recognise the significance of collaborative working. Teamwork is
essential in multidisciplinary environments. Effective communication is
essential, and all staff must ensure that they communicate in a manner which
enable the sharing of information in an appropriate manner.
Service
delivery
Staff
at PGPA must adhere to the information contained within the practice and PGPA
policies and regional directives, ensuring that protocols are adhered to at all
times. Staff will be given detailed information during the induction proves
regarding policy and procedure.
Security
The
security of PGPA is the responsibility of all personnel. Staff must ensure that
they remain vigilant at all times and report any suspicious activity
immediately to their line manager. Under no circumstances are staff to share
codes for the door locks with anyone and they are to ensure that restricted
areas remain effectively secured.
Professional
conduct
At
PGPA, staff are required to dress appropriately for their role. Clinical staff
must dress in accordance with their role.
Leave
All
personnel are entitled to take leave. Line managers are to ensure all of their
staff are afforded the opportunity to take a minimum of PGPA days leave each
year and should be encouraged to take their leave entitlement.
