Job summary

In this innovative post you will connect people who are in employment but off sick due to a health condition and at risk of losing employment, with a range of opportunities that support an early intervention approach around mental health and wellbeing.

As part of the exciting NHS England programme, Get Britain Working, you will play a vital role by using data to proactively identify people with a Fit note and at risk of leaving employment. You will have a passion for supporting change across the system, navigating and connecting people and services to reduce heath inequalities and widen access to support.

Working across the programme with Primary care NHS, South West Yorkshire Partnership Foundation Trust, Calderdale Metropolitan Borough Council and the Voluntary, Community and Social Enterprise sectors, you will connect people in line with their needs and preferences to interventions that best support them to return to and stay well at work. You will be a key point of information, navigation and contact for all providers to ensure seamless signposting and advice.

Main duties of the job

Undertake work in line with Accelerator Get Britain Working priorities.

Use clinical system searches to proactively identify patients that may benefit from additional employment support, targeted for mental health and MSK issues.

Build and maintain and understanding of existing employment and navigational services available, together with their remit and referral criteria to support collaborative working.

Work collaboratively with all relevant local organisations, in line with direction from the Accelerator programme.

Keep quality patient record notes and coding.

It is vital that you have a strong awareness and understanding of when it is appropriate or necessary to refer people back to other health professionals/agencies, when what the persons need is beyond the scope of the Employment Support Navigator role e.g., when there is a physical or mental health need requiring a qualified practitioner.

Promoting social prescribing, its role in self-management and active care navigation, and the wider determinants of health with practices and wider.

Build relationships with key staff in GP practices, attending relevant meetings, becoming part of the wider network team, giving information and feedback on the programme.

Be proactive in developing strong links with all local statutory and third sector organisations, working in partnership with local agencies to raise awareness of the programme.
