Job summary

We require candidates to you have maintained professional registration with the Faculty of Physician Associates or other similar recognised body. Main duties of the job

You will support the PCN on a day-to- day basis, acting within your professional boundaries, providing care for patients from initial history taking, clinical assessment, diagnosis, treatment, and evaluation of their care under the supervision of a GP. You will demonstrate safe clinical decision-making and expert care for patients in consultation with the duty GP. You will work with our primary care teams to meet the needs of patients whilst receiving mentorship and supervision by designated senior medical personnel.

About us

A newly formed PCN consisting of four like-minded practices working together across the City to enhance the service we provide to our patients.

Whilst PCN arrangements will cease from 31st March 2024 we fully expect the infrastructure to stay, and support for staff will continue. It will be business as usual because there is enough demand, and we will need to deliver local and resilient solutions to meet the needs of the patient population.

Job Summary

Working as part of the primary care team you will work across the PCN GP practices providing advice for the presenting patient. This will include history taking, clinical assessment, diagnosis, treatment and evaluation of care.

Key Areas of Responsibility

Provide the first point of care contact for patients presenting with undifferentiated, undiagnosed problems by utilising history taking, physical examinations and clinical decision-making skills to establish a working diagnosis and management plan in partnership with the patient (and carers).

Offer specialised clinics (where competent), but not limited to family planning, baby checks, COPD, asthma, diabetes and anticoagulation.

Provide health promotion and prevention advice, alongside analysing and actioning diagnostic test results.

Develop integrated patient-centred care.

Promote evidence-based practice and partake in clinical audits, significant events reviews and other related activities.

Participate fully in the duty rota across the PCN, including face to face, remote consultations and homes visits for both emergency and routine matters and long-term conditions.

Working under the supervision of a doctor as part of a medical team.

Management of workload and personal development

Act on own initiative in prioritising and managing workload, in respect of clinical, managerial, and administrative areas, as necessary.

Maintain own personal and professional development as agreed at your annual review utilising a reflective approach to practice, operating under appropriate clinical supervision.

  • Has a detailed knowledge and understanding in practice of applying Caldicott Principles, Data Protection Act 2018 and appropriately protects an individuals personal data, disclosing where appropriate only
