Job summary

We have a fantastic opportunity to join our innovative team of Care Co-ordinators working in the Tame Valley Primary Care Network (PCN) in Stockport.

As a Safeguarding and Long Term Conditions Care Co-ordinator, you will be supporting all key safeguarding activity across the PCN; supporting the PCN manager and associated practices by co-ordinating activity and providing an efficient, well organised administrative and operational support to the clinicians and managers in the network to ensure effective timely delivery of the PCN objectives. This positionwill be integral in overseeing the interdisciplinary care of each patient, and will be responsible for co-ordinating a package of care and support from a variety of specialists who may be working with the patient.

Main duties of the job

This role provides comprehensive administrative and operational support across safeguarding and long-term condition management within the Tame Valley Primary Care Network (PCN). Key safeguarding responsibilities include processing correspondence from external agencies, maintaining safeguarding registers, supporting multi-agency meetings, facilitating staff training, and ensuring compliance with audits and assurance processes. The role also involves embedding learning from safeguarding reviews, escalating concerns appropriately, and maintaining up-to-date directories of safeguarding contacts.

In relation to long-term conditions, the postholder will oversee recall systems, conduct EMIS searches, manage recall schedules, and support the delivery of key performance targets such as QOF, IIF, and enhanced services. They will ensure consistent processes across practices, maintain accurate coding, and prepare patient communications. More broadly, the role requires flexible working across sites, providing high-quality administrative support, and undertaking additional duties as directed by the Clinical Director or PCN Lead Manager.

Viaduct Care CIC is the company structure for Stockports GP Federation and represents all of its local GP Practices. Covering a patient population of circa 300,000 practices are split into 6 PCNs with each serving a population of around 30,000-50,000 patients.

Viaduct Care represents the collective voice and interests of its member practices and as a key stakeholder in Stockport Together aims to influence and support the design and delivery of major service and system changes by being a strong and effective partner with other major service providers.

o Smooth workload across the year by managing recall schedules.

o Support achievement of QOF, IIF, Vaccinations & Immunisations, and other Enhanced Service Contract requirements.

o Monitor Population Manager achievements and liaise with practices to address underperformance.

o Enter and amend SNOMED codes in line with defined protocols.

o Prepare patient letters and information leaflets as required.

General

o Provide efficient, well-organised administrative and operational support to PCN practices.

o Work flexibly across practices, remotely, or in community settings as required.

o Undertake other duties reasonably required by the Clinical Director or PCN Lead Manager.

Qualifications

Essential

  • Achieved grade C or above, in English and Maths GCSE or equivalent.
  • PCI Accredited Care Coordinator training (or be willing to work towards this)

Desirable

  • NVQ Level III (Health and Social Care)
  • Formal training in safeguarding or long-term condition management

  • Previous experience working with the general public.
  • Experience of working autonomously and part of a team.
  • Ability to recognise and respond appropriately to risk and safeguarding concerns.
