Job summary

Health and Wellbeing Coaches are new to Primary Care and we have recently recruited a number of these roles across the borough of Stockport.

We know that there is a high demand for emergency and urgent care services and this is putting pressure on NHS organisations and staff and seeing an increase in the attendances of children under 5 years old with non-urgent problems contributes to this demand.

The Childrens Health Coach will support the Stockport East and South PCN (Primary Care Network) to reduce GP workload by reviewing Emergency Department attendances for children under 5 years old. The Health Coach will offer this support to 8 GP practices within the PCN.

Main duties of the job

The Childrens Health Coach will play an important role within the PCN to support with delivering targeted education to parents on how to manage non-urgent childhood illnesses and whether there are alternatives to attending the Emergency Department.

Job responsibilities

Main Roles & Responsibilities:

  • The aim of this role is to reduce the number of non-urgent attendances to ED for children under 5 years old.
  • The role will involve contacting parents and discussing why they attended ED and coaching parents, where appropriate to explore alternatives to support.
  • To complete assessments where necessary i.e. Identify support needs and options for support pathways based on individual needs.
  • The role will also involve contacting new/vulnerable parents to provide check ins and offer advice about suitable support services.
  • The role requires a caring, empathetic, and patient focused individual who enjoys supporting parents and families.
  • The role will involve working alongside a range of professionals, such as GPs, health visiting services and paediatricians.
  • To support clinical teams with support as appropriate:
  • The post holder will be expected to build links with local childrens services/groups.
  • To run occasional parent education sessions to offer general information and advice about illnesses.
  • Ensure all relevant patient information, including historical information, compliance issues, are managed, and symptoms are provided and escalated appropriately.
  • To work in partnership with PCN Safeguarding Care Co-ordinators and Social Prescribers.
  • To provide regular performance reports (including analysis) in relation any on-going work streams.
  • To support with the development of parent education materials
  • To support the Care Co-ordinator Manager and Service Lead in developing this new service
  • Share ideas with colleagues to improve care and suggest areas for innovation.
  • Participate in audit activities being undertaken across the PCN.
  • Contribute to the improvement of service by reflecting on own practice and supporting that of others.
  • Adhere to legislation, policies, procedures, and guidelines both local and national.
  • Regularly attend workplace and staff engagement meetings and contribute positively to discussions about the improvement of care.
  • Maintain patient records.

Standard Clauses

Performance Management

Objectives for the post will be the subject of overall agreement and regular review between the post-holder and their line manager. This will via regular supervision and the organisations Performance Development Review (PDR) Aspire and Perform process.

Confidentiality and Compliance with the Data Protection Act 2018

The post holder must maintain confidentiality regarding information about patients, staff and other Viaduct Care business in accordance with the Data Protection Act 2018.

All employees of Viaduct Care must not, without prior permission, disclose any information regarding patients or staff. In circumstances where it is known that a member of staff has communicated information to an unauthorised person, those staff will be liable to dismissal. Moreover, the Data Protection Act 2018 also renders an individual liable to prosecution in the event of unauthorised disclosure of information.

Health and Safety

All employees must be aware of the responsibilities placed on them under the Health and Safety at Work Act 1974 to ensure that the agreed safety procedures are carried out to maintain a safe working environment for patients, visitors and employees.

Review and ensure systems are in place for the proactive management of risk registers.

Infection Control

All employees of Viaduct Care are required to contribute to the management of infection control and be proactive in awareness raising and prevention.

Ensure that all guidelines, protocols and procedures undertaken within the service are in line with evidence-based practice for infection control.

Safeguarding Children and Adults

All staff are required to act in such a way that always safeguards and promotes the health and well-being of children and vulnerable adults. Familiarisation with and adherence to the policies and procedures of the Local Safeguarding Boards (Children and Adults) and those of Viaduct Care is an essential requirement of all staff as is participation in mandatory safeguarding training in accordance with individual roles and responsibilities.

  • Achieved grade C or above, in English and Maths GCSE or equivalent.
  • Formal training in working with children or families.
  • NVQ Level III (Health and Social Care) or equivalent or equivalent experience.

Desirable

  • Coaching Qualification(s)/mentoring L3
