Main duties of the job
Hospice at Home provide a reactive service to those deemed to be in their final weeks of life requiring specialist palliative care review and intervention. The service is supplementary to, and works alongside, other health care services within North Hampshire so that people who use our service receive co-ordinated and person-centred care.
You will be responsible for the assessment of care needs and the development, implementation and evaluation of programmes of care. You will develop, implement and evaluate care plans in conjunction with patients, relatives and other professionals ensuring individual physical and psychological needs are met.
Opportunities exist to rotate between services to further develop your knowledge and skills in palliative care and end of life and enhance awareness of the patient and family experience of end-of-life care.
By joining our team, you will get protected study time, clinical supervision and regular 1:1 with management to review development needs.
The Hospice at Home team cover shifts over a 7-day working week. You will also be expected to be able to attend additional training throughout the rotation including mandatory training, local bespoke training and university accredited courses.
