Job summary
This exciting opportunity has arisen within the Bristol Mental Health Partnership, which involves collaborative working between a number of organisations to include Second Step and Avon and Wiltshire Partnership Trust (part of the National Health Service). The North Crisis Team is one of three established crisis teams in the Bristol area, with the key remit of our service being to support people in avoiding hospital admissions, provide intensive home treatment to those at risk of hospital admission and to support people in their transition home following a stay in an acute psychiatric hospital.
Main duties of the job
Facilitated Discharge Workers are responsible for acting as the link for inpatient wards, community teams and supporting safe discharges, while maintaining a safe level of risk. You will liaise with service users and ward staff prior to discharge to identify potential barriers and reduce the length of admission.
