Job summary

The Intensive Outreach Service (IOS) supports individuals across Bath and North East Somerset, Swindon and Wiltshire (BSW) who are experiencing severe and enduring mental health challenges. We work alongside the AWP Intensive Team to deliver person-centred, trauma-informed and culturally inclusive support, helping individuals move toward stability, independence and engagement in their communities. IOS is part of the Mental Health and Wellbeing Partnership, a collaboration between Second Step, Alabar, Missing Link and Nilaari, working in partnership with NHS providers and Primary Care Networks across BSW.

Main duties of the job

As a Recovery Navigator, youll be embedded in a multi-disciplinary team, delivering intensive, personalised support.

Recording your work using client-focused, trauma-informed approaches. Advocating for inclusion and equality, especially for marginalised and minoritised individuals.

Person Specification

Experience

Essential

  • A minimum of two years significant experience working with people with mental health needs, or people with complex needs
  • Proven experience of working with people with multiple needs (e.g. mental health, homelessness, alcohol/drug dependency long term conditions)
  • Proven experience of carrying out needs and/orrisk assessments and the support planning process
  • Experience of working with a number of individuals with competing needs and priorities

Desirable

  • Proven experience of mental health issues and/or services, either as a user, carer or supporter
